Job Summary

Ennoble Care is seeking a compassionate, detail-oriented Licensed Practical Nurse (LPN) to join our palliative care team in a remote capacity. The Remote Palliative Care LPN will support patients with serious or life-limiting illnesses by providing ongoing clinical coordination, symptom-management support, patient and caregiver education, and communication with the interdisciplinary care team.

This position is ideal for an LPN who is passionate about patient-centered care and can effectively build trusting relationships with patients and families through telephone and virtual communication.

Key Responsibilities

  • Manage scheduled telephone and/or virtual patient check-ins under the direction of the supervising RN/provider.
  • Conduct high-acuity patient chart reviews to assess hospice clinical appropriateness.
  • Monitor and document patient-reported symptoms, changes in condition, medication concerns, and care needs.
  • Provide education to patients and caregivers regarding medications, symptom management, disease processes, and available supportive services.
  • Escalate changes in patient condition or urgent concerns to the appropriate RN, provider, or interdisciplinary team member.
  • Assist with care coordination among patients, caregivers, physicians, nurses, pharmacies, home health agencies, hospice/palliative care providers, and other healthcare professionals.
  • Follow established clinical protocols, care plans, and organizational policies.
  • Provide emotional support and compassionate communication to patients and families navigating serious illness.
  • Assist with medication reconciliation and identify potential medication-related concerns for review by the appropriate clinician.
  • Maintain accurate, timely, and thorough documentation in the electronic medical record (EMR).
  • Participate in interdisciplinary team meetings and communicate relevant patient updates.
  • Assist with referrals, follow-up appointments, supplies, and community resources as appropriate.
  • Protect patient privacy and maintain compliance with HIPAA and applicable federal and state regulations.
  • Perform other duties within the LPN/LVN scope of practice as assigned.

Qualifications

  • Current, unrestricted LPN license in the applicable state(s).
  • Graduate of an accredited practical nursing program.
  • Prior experience in palliative care, hospice, home health, oncology, geriatrics, chronic disease management, or a related setting preferred.
  • Strong clinical assessment, communication, organization, and documentation skills.
  • Comfortable providing patient support by telephone and/or virtual platforms.
  • Ability to recognize changes in patient condition and appropriately escalate concerns.
  • Proficiency with electronic medical records and standard computer applications.
  • Strong interpersonal skills and the ability to communicate with patients and families with empathy and professionalism.
  • Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
