This position plays a crucial role in providing direct nursing care and supporting safe, efficient clinic operations within a PACE geriatric clinic. The PACE Clinic Registered Nurse supports participant-centered care through clinic-based assessment, treatment support, education, vaccine and preventive health programs, supply readiness, documentation, interdisciplinary team collaboration, and cross-coverage for in-home nursing or nursing case management needs as indicated during program growth and scale.

  • Develop, implement, and coordinate individualized nursing care for PACE participants in the clinic setting using established procedures, protocols, and policies.
  • Conduct clinic-based nursing assessments, monitor changes in participant condition, coordinate follow-up needs, and support diagnostic testing and treatment plans as directed by the provider or interdisciplinary team.
  • Provide direct clinic nursing care, including vital signs, medication administration, wound care, injections, immunizations, treatment support, specimen collection, and assistance with medical procedures within scope of practice.
  • Work closely with physicians, nurse practitioners, clinic staff, rehabilitation staff, social workers, and other PACE team members to support comprehensive, coordinated, and compassionate participant care.
  • Educate participants and caregivers on treatments, medications, self-care, chronic disease management, vaccine recommendations, and prevention strategies to support participant engagement and health outcomes.
  • Support vaccine and preventive health programs, including vaccine planning, participant outreach, screening, administration, documentation, inventory coordination, and follow-up in collaboration with the clinic and interdisciplinary team.
  • Monitor clinic supplies, medications, vaccines, equipment, and other clinical resources to support readiness for participant care; communicate supply needs, expiration concerns, equipment issues, or workflow barriers to the appropriate leader or designee.
  • Respond to complex clinical situations using nursing judgment, established escalation pathways, and interdisciplinary collaboration to promote participant safety and timely care.
  • Fulfill all clinic and nursing documentation requirements in a timely, accurate, and complete manner, including participant charting, medication and vaccine documentation, care coordination notes, incident reporting, and other required records.
  • Act as a clinical resource to clinic staff and assist physicians, nurse practitioners, and other licensed professionals with participant care needs, clinic flow, and follow-up activities.
  • Actively participate in the PACE interdisciplinary team as indicated by participant needs, offering nursing expertise in care planning, problem solving, risk identification, follow-up planning, and process improvement.
  • Provide cross-coverage for in-home nursing visits or nursing case management activities as indicated by participant needs, staffing model, program growth, and scale, consistent with licensure, scope of practice, and organizational policy.
  • Participate in team meetings, caregiver groups, vaccine clinics, quality initiatives, participant events, and other special activities as needed to support clinic operations and participant care.
  • Provide on-call or urgent clinical coverage for direct participant care with other staff as needed and consistent with operational requirements.
  • Foster a safe and supportive environment conducive to the professional development of healthcare professionals.

Supporting Responsibilities:

  • Meet department and company performance and attendance expectations.
  • Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
  • Perform other duties as assigned.

Work Experience: A minimum of 3 years’ nursing or behavioral health experience with varied health care exposure and experience required. Experience in working with frail, older adults in state programs required.

Education, Certificates, Licenses: A Registered Nurse with a current, unrestricted state license and an associate’s degree is required. Valid CPR certification required at time of hire or within three months of the hire.

Knowledge: Thorough knowledge of community services, providers, vendors and facilities available to assist members. Ability to use computerized systems for data and document recording and retrieval. Maintain current clinical knowledge base. Excellent interpersonal and communication skills. Ability to work independently and as part of a team. Proficiency in electronic medical records systems preferred.

Each member of the PACE organization's staff that has direct contact with participants must meet the following conditions:

  • Be legally authorized (for example, currently licensed, registered or certified if applicable) to practice in the State in which he or she performs the function or action.
  • Only act within the scope of his or her authority to practice.
  • Have 1 year of experience working with a frail or elderly population
  • Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.
  • Be medically cleared for communicable diseases before engaging in direct participant contact.

Environment: Work inside in general office and clinical settings with ergonomically configured equipment. Travel is required approximately 10% of the time.

Base Range:

$70,950.00 - $106,424.99

Physical Requirements: Work is performed in a clinic setting with typical healthcare exposures, including contact with bodily fluids, infectious diseases, and cleaning chemicals; PPE is provided and required. The role involves stooping, bending, sitting, standing, and moving between clinical areas for extended periods. May assist participants with mobility, including guiding, supporting, escorting, and occasional transfer assistance using proper body mechanics. Must be able to lift, push, pull, or carry up to 35 pounds occasionally and perform frequent repetitive motions such as typing and operating medical or office equipment. Some positions require occasional travel to PACE sites, participant homes, or external appointments. Must be able to speak clearly, understand verbal instructions, and interact professionally with participants, caregivers, and staff.
