POSITION DESCRIPTION
Ennoble Care is seeking a Medical Director, Geriatric Medicine, to provide direct medical care to older adult patients across clinics, long-term care facilities, assisted living communities, and patients' homes in the Youngstown, Ohio, area. Reporting to the Regional Chief Medical Officer (RCMO), this role combines hands-on geriatric clinical practice with support for the region's broader medical direction, quality, and care-coordination efforts. The position is available on a flexible schedule, ranging from a minimum of two days per week to full-time, based on candidate availability and business needs – an ideal fit for a physician who wants to bring deep geriatric expertise to a growing home-based care organization.
RESPONSIBILITIES
● Direct Patient Care: Provide direct medical care to a panel of older adults across clinic, long-term care, assisted living, and home settings, delivering high-quality, patient-centered geriatric medicine.
● Geriatric Assessment: Conduct comprehensive geriatric assessments, including medical, functional, cognitive, and psychosocial evaluations, to guide individualized care planning.
● Chronic Disease & Medication Management: Diagnose and manage complex, chronic conditions common in older adults – including dementia, frailty, cardiovascular disease, osteoporosis, and diabetes – and monitor and adjust medications with attention to polypharmacy and age-related pharmacologic considerations.
● Treatment Planning & Education: Develop individualized treatment plans emphasizing prevention, health maintenance, and patient and caregiver education.
● End-of-Life & Palliative Care Planning: Guide patients and families through end-of-life care planning, advance directives, and palliative care coordination when appropriate.
● Care Coordination: Coordinate care with Ennoble Care's multidisciplinary team, including nurses, social workers, therapists, pharmacists, and other specialists, and communicate effectively with patients,
families, and caregivers on goals of care and quality-of-life concerns.
MEDICAL DIRECTOR, GERIATRIC MEDICINE
● Team Collaboration: Collaborate with and support nurse practitioners and physician assistants delivering care to the shared patient panel.
● Medical Direction Support: Support the Regional Chief Medical Officer in the development and reinforcement of clinical protocols and best practices for geriatric care, and contribute geriatric-specific
clinical expertise to case reviews and provider training as needed.
● Quality & Compliance: Participate in quality improvement initiatives and clinical research as directed by the RCMO, and maintain timely, accurate clinical documentation in compliance with regulatory, ethical,
and organizational standards.
● Continuing Education: Maintain continuing medical education relevant to geriatric and home-based primary care.
QUALIFICATIONS
● Medical Licensure & Credentials: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution, with board certification (or eligibility) in Internal Medicine or Family
Medicine and an active, unrestricted Ohio medical license (or ability to obtain).
● Geriatric Clinical Expertise: Strong clinical skills in diagnosing and managing complex, age-related conditions; fellowship training in Geriatric Medicine is preferred.
● Care Setting Experience: Experience in long-term care, skilled nursing facilities, home health, or palliative care is strongly preferred.
● Interdisciplinary Collaboration: Familiarity with interdisciplinary care models and comfort working closely with nurse practitioners, physician assistants, and other members of the care team.
● Geriatric Assessment Tools: Knowledge of geriatric syndromes, mobility and fall-risk assessment, and cognitive evaluation tools.
● Program Standing: Not excluded from participation in Medicare or other Federal Health Care Programs.
● Communication & Empathy: Excellent verbal and written communication skills, with strong empathy and teamwork abilities.
● Mobility: Valid driver's license and ability to travel locally to patients and facilities in the Youngstown, Ohio area.
