Midtown Oaks Post Acute is a 100-bed skilled nursing and post-acute rehabilitation facility dedicated to providing compassionate, personalized care for our residents. We specialize in short-term rehabilitation, skilled nursing services, and long-term care, helping residents recover, regain independence, and achieve their highest level of well-being.

Position Summary

The Licensed Vocational Nurse (LVN) provides direct nursing care to residents and supervises Certified Nursing Assistants (CNAs) to ensure the highest standards of clinical care. The LVN works closely with physicians, nursing leadership, residents, and interdisciplinary team members to promote positive outcomes and exceptional resident experiences.

Essential Responsibilities

Resident Care

• Administer medications and treatments as prescribed by physicians.
• Monitor resident conditions and promptly report changes to nursing leadership and physicians.
• Perform skilled nursing procedures including wound care, catheter care, tube feedings, suctioning, and other clinical interventions as appropriate.
• Take and record vital signs, observations, assessments, and resident responses to treatment.
• Participate in resident admissions, transfers, and discharge processes.

Documentation & Compliance

• Maintain accurate, timely, and compliant clinical documentation.
• Transcribe physician orders and update resident records as needed.
• Review medication and treatment records for accuracy and completeness.
• Ensure compliance with facility policies, state regulations, and federal guidelines.

Leadership & Team Collaboration

• Supervise and support nursing assistants during daily care delivery.
• Collaborate with physicians, therapists, social services, and interdisciplinary team members.
• Participate in care planning meetings and resident care conferences.
• Assist in maintaining a safe, compassionate, and resident-centered environment.

Qualifications

Required

• Current, active California Licensed Vocational Nurse (LVN) license in good standing.
• Graduate of an accredited LVN/LPN nursing program.
• Current CPR Certification.
• Strong clinical assessment and communication skills.
• Ability to work effectively in a fast-paced healthcare environment.

Preferred

• Skilled Nursing Facility (SNF) experience.
• Post-Acute Rehabilitation experience.
• Experience with PointClickCare (PCC) documentation system.
• New graduates are encouraged to apply.
