Job Title: Licensed Practical Nurse - Acute General Medicine

SUMMARY: The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans. The Licensed Practical Nurse is a key member of the care delivery team. The Licensed Practical Nurse communicates the needs of the patient and the team; collaborates with the multidisciplinary team, takes direction from the RN and delegates appropriately to the other members of the team.

The Acute General Medicine Unit (4800) provides specialized care for patients with substance use disorders, mental health conditions, and a wide range of complex medical diagnoses. This unit offers the opportunity to develop and refine advanced clinical skills in areas such as cardiac care with telemetry monitoring, diabetes management, renal disease and dialysis, pulmonary conditions, and the management of patients with multiple drips, drains, and catheters.

RESPONSIBILITIES:

· Patient Care & Service - Promote and restore patients' health by completing the nursing process; collaborate with physicians and multidisciplinary team members; perform various treatment procedures; provide physical, educational and emotional support to patients, friends and families; supervise assigned team members.

· Medication Administration & Reporting - Administer medication and give/receive relevant reports, both written and verbal from/to appropriate staff as per departmental protocols.

· Documentation - Ensure concise, pertinent and complete documentation using computerized medical record process.

· Preparation - Ensure clinical areas are properly supplied and exam rooms are available and stocked.

REQUIRED QUALIFICATIONS:

· Graduate from an accredited School of Practical Nursing or equivalent required.

· Current licensure in State of New York as a Licensed Practical Nurse.

PREFERRED QUALIFICATIONS:

· Strong communication, customer service, and interpersonal skills highly desired.

PAY RANGE:

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
