The Field Care Manager, Behavioral Health 2 assesses and evaluates member's needs. We do this to achieve or maintain wellness state. You will guide members/families toward resources and facilitate interaction with them. These resources are appropriate for the care and wellbeing of members. The Field Care Manager, Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.The Field Care Manager, Behavioral Health 2 employs a variety of strategies and techniques to manage a member's physical, environmental and psycho-social health issues. Resolve barriers that hinder care. Ensure patient is progressing towards desired outcomes by overseeing patient care through assessments or evaluations. May create member care plans. Understand department, segment, and organizational strategy and operating goals, including their linkages to related areas. Regarding own work methods, occasionally in, and receive guidance where needed. Follow established guidelines/procedures.

Required Qualifications
- Reside in the State of Indiana
- Master's degree in social work (Master of Social Work) or therapy
- Current Indiana Licensed Social Worker or Licensed Clinical Social Worker license·
- One (1) year of experience providing case management services to individuals who are older adults and individuals with physical or developmental disabilities or individuals determined to have a serious mental illness (SMI)
Preferred Qualifications
- Bilingual (English/Spanish) or (English/Burmese)
- Prior nursing home diversion, long-term care, disease management, or case management experience·
- Manage Home and Community-based Services waivers (HCBS dual roles only)·
- Prior experience with Medicare & Medicaid recipients· Experience working with a geriatric population
- Knowledge of community health and social service agencies and additional community resources

This position may be eligible for a $5,000 Sign on Bonus
Workstyle: Combination remote work at home and onsite member visits
Location: Must reside in Indiana
Hours: We require the ability to work a 40-hour work week, Monday through Friday 8:00 AM to 5:00 PM, and we may request over-time to meet our needs.
Travel: Must be willing to commute about 70% to meet with members.
We may require On Call-Telephonic on call for an occasional night or weekend.
Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
