Major Responsibilities:

• Provides strategic and operational leadership to teammates responsible for Community Health Needs Assessments, Implementation Strategies, and Community Impact priorities across the enterprise.
• Advance enterprise community impact strategies that address upstream drivers of health, including affordable housing, food security, economic opportunity, and other priority areas identified through community health needs assessments.
• Supports executive-level prioritization, coordination, and follow-through by helping translate strategic direction into action, preparing decision-ready materials, aligning stakeholders, and ensuring timely progress on key enterprise priorities.
• Establishes and oversees team goals, workplans, timelines, and deliverables to ensure effective execution of enterprise community health and impact strategies.
• Partners with senior leaders and cross-functional stakeholders to align team activities with enterprise priorities, clinical strategies, philanthropic efforts, policy considerations, and community partnership opportunities.
• Ensures appropriate governance, coordination, reporting, and communication structures are in place to support consistent execution and visibility of community health and impact initiatives.
• Monitors progress, evaluates outcomes, identifies risks and opportunities, and provides recommendations to senior Community Health & Impact leadership to strengthen team effectiveness and enterprise impact.
• Promotes a collaborative, inclusive, and mission-aligned team culture that supports professional growth, continuous improvement, and shared accountability for community health outcomes.

Licensure, Registration, and/or Certification Required:

• N/A

Education Required:

• Bachelors degree in public health, epidemiology, social work, community development, public policy or a related field addressing the social drivers of health required.

Experience Required:

• 7 years of experience addressing and improving the social drivers of health, community health strategy, population health initiatives, or related areas.

Knowledge, Skills & Abilities Required:

• Demonstrated ability to lead, coach, develop, and manage teammates, including setting priorities, supporting performance, and fostering accountability for results.
• Knowledge of Community Health Needs Assessment and Implementation Strategy processes, including community engagement, data interpretation, priority setting, reporting, and implementation oversight.
• Experience with community investment, philanthropic strategy alignment, and policy and/or advocacy work at local, state, or federal level.
• Ability to provide strategic guidance and operational direction for enterprise-level community health and impact initiatives in a complex, matrixed environment.
• Strong executive communication skills, including the ability to prepare decision-ready materials, synthesize complex information, and present recommendations to senior leaders and stakeholders.
• Ability to establish governance structures, coordinate cross-functional work, monitor progress, identify risks, and ensure timely follow-through on key priorities.
• Demonstrated ability to build and maintain effective relationships with internal leaders, external partners, community stakeholders, and cross-sector collaborators.

Physical Requirements and Working Conditions:

• This position requires occasional travel and may involve exposure to weather and road conditions.
• Operates equipment necessary to perform job functions.
• This position may function virtually and requires the ability to work from a non-Advocate Health location.

Preferred Education:

• Masters degree in public health, epidemiology, social work, community development, public policy or a related field addressing the social drivers of health required.
