• Position: Clinical Nurse Coordinator – Outpatient Wound Care - Days
• Location: Swedish Hospital, Chicago, IL
• Part-Time (40 hours per 2-week pay period)
• Hours: M-F 8:00 am - 4:30 pm

What you will do:

The outpatient Wound Center Clinical Coordinator is a Registered Nurse designated to facilitate the functioning of the unit, under the direction of the Clinical Nurse Manager . The Coordinator is directly responsible for coordinating care for the wound clinic patient populations. The Coordinator demonstrates excellent clinical skills and acts as a resource to all staff in the clinic. The Coordinator should be capable of providing direct patient care, however this role will not be factored in to daily staffing module. Major components of this role include educating, coordinating, delegating, communicating and thinking critically on clinical and operational issues.

• Functions as the educational and coordinating resource for the nursing staff in the Wound Care Department.  Serves as primary navigator in the clinic.
• Coordinates patients assignments, covers for staff, educates in the moment and ensures compliance with required documentation for procedures.
  • Works directly with the Wound Care Department to identify the care needs for  patients throughout the continuum of care
  • Directs flow of the outpatient Wound Care Department
  • Develops and maintains competence to provide direct care to the wound care department patient population
  • Provides direct care if needed and assigns appropriate staff to participate with this care
  • Directly mentors / educates staff and monitors competence on an ongoing basis
  • Acts as a liaison between the wound care staff and the wound care provider/residents and other departments to develop and ensure a smooth process for the safe and effective care of patients in the Wound Care Department.
  • Manages the phone calls to the department and documents on returned calls to patients
  • Manages DME orders for patients receiving care in the Wound Care Department
  • Manages benefit verification for advanced modalities; ensures patients have complete and correct documentation prior to BV submission
• Acts as additional chain-of-command resource of the Wound Care Center covering select duties for the Clinical Nurse Manager during his/her absence, working effectively with the nursing and other departmental leadership
• Alternate to charge reconciliation and tissue bank tracking
• Acts as a resource to nursing staff in problem solving resolution of patient care issues
• Provides some supervision and evaluation of clinical skills of nursing staff
• In addition to coordinate the care of wound care patients:
• Collaborates with admitting, the Bed Coordinator, and/or the Supervisor to facilitate the plan for timely placement of admissions from the wound care center
• Ensures adequate and appropriate coverage for the unit that considers census, acuity and RN patient ratio.  Plans for and makes unit assignments based on staffing, competency of staff, coverage of following shifts
• Delegates assignments to appropriate RN based on criteria and adjusts patient care assignments as necessary
• Delegates work to non-professional staff (tech, Unit Secretary)
• Counsels professional and non-professional staff and gives feedback when necessary
• Coordinates staff and their duties during direct patient care - routine and emergency

What you will need:

• Education: Graduate of an approved school of nursing. Bachelor / Master of Nursing-prepared preferred
• Certification: Current license as a Registered Nurse in Illinois. American Heart Association BLS
• Experience: Minimum of two years experience as a Wound Care Staff Nurse
• Unique or Preferred Skills: Demonstrates a commitment to the mission of Swedish Hospital and demonstrates a positive service orientation and adheres to all responsibilities and standards of the Hospital. Demonstrates a team spirit in daily activities. Displays an enthusiasm for patient care and a commitment to positive customer service. Demonstrates good interpersonal communication skills and professional behavior. Promotes Hospital/Nursing Unit Quality Improvement goals. Demonstrates effective leadership skills, working effectively with clinical staff, delegating to their individual skill levels, and mentoring appropriate behavior. Works effectively with physician staff by showing an enthusiasm for learning and implementing their care plans
