Case Manager – Housing Support Program 

Maximum term contract until 31 December 2027  

Full-time | Darwin, NT

This role offers the chance to walk alongside people who are working to rebuild stability, safety and connection in their lives. Many participants face challenges such as homelessness, mental health concerns, AOD use, justice involvement and social isolation. As a Case Manager, you help create a supportive environment where people feel heard, respected and empowered to pursue their own goals.

About the role 

You will provide case management support to participants in the Housing Support Program, working collaboratively to develop case plans that reflect each person’s strengths, needs and selfdetermined goals. You will assist participants to attend appointments, build capacity, access community supports and engage with TSA programs such as Towards Independence and Doorways. 

An important component of this role is providing outreach case management support to clients in their private/public housing. This requires strong situational awareness, sound risk assessment and safety practices, and effective time-management skills when working independently outside the office. 

This role also involves completing intake and assessment, maintaining accurate records, responding to potential risks, and advocating for participants with government and nongovernment agencies. You will contribute to a culturally safe, traumainformed and inclusive service environment that supports people with low to mild mental health needs and occasional periods of higher intensity support. 

This maximum term position is contracted until 31 December 2027 and sits within the Homelessness team, reporting to the Supported Housing Senior Team Leader. Pay is in accordance with a SCHADS Level 4. 

How you will make an impact 

  • Support participants to achieve selfdetermined goals through collaborative case planning 

  • Assist participants to attend appointments, build capacity and engage with community supports 

  • Complete intake and assessment to develop shared understanding of strengths and needs 

  • Maintain accurate, uptodate records in line with TSA standards 

  • Take preventative action in potential highrisk situations in accordance with policy 

  • Advocate for participants with government and nongovernment agencies 

  • Build effective relationships that encourage selfdetermination and participation 

  • Contribute to a safe, respectful and inclusive service environment 

What you will bring 

  • Degree qualification in Social Work, Mental Health, Welfare or Community Development, or relevant experience with a minimum Certificate IV in Community Services 

  • Experience supporting people with homelessness, AOD dependence, mental health concerns or justice involvement 

  • Ability to engage respectfully with people from diverse cultural backgrounds, including Aboriginal and Torres Strait Islander communities 

  • Strong communication skills and the ability to create shared understanding 

  • Good organisational skills and experience managing a case load 

  • Resilience, adaptability and commitment to culturally safe, inclusive practice 

  • First Aid Certificate 

  • NT Working with children check or willingness to apply 

  • NT Driving Licence  

What we offer 

  • $46.70 – $50.25 hourly + superannuation based on experience level and qualifications, exclusive of penalties, allowances, NFP salary packaging
