About the Role

An exciting opportunity exists for a Registered Nurse or Grade 2 Allied Health Professional or to join the Transition Care Program at St Vincent’s. The successful applicant will be responsible for providing high quality, evidence-based Case Management support to aged care clients in Transition Care, working as part of a supportive, multi-disciplinary team. The role is based at St George’s Hospital in Kew and works as part of a close-knit, multi-disciplinary team. The Case Manager - Transition Care Program will assume a key role in the care coordination and case management of clients within the program. This will include conducting comprehensive holistic health assessments, coordinating health and social care needs, monitoring achievement of client goals, facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital and community services. The Case Manager may also be involved in establishing ongoing community support services and identification of residential care options. This is a great opportunity for an enthusiastic Allied Health Professional or Nurse with aged care experience to join a cohesive and progressive integrated team, committed to excellence in clinical practice, research and professional development. The incumbent must hold, or be currently eligible for, membership with the relevant professional body. Relevant postgraduate qualifications will be highly regarded.

Classifications for this position will be: Social Worker Grade 2, SC21-SC24; Occupational Therapist VF6-VF9; Physiotherapist VB1-VB4; Speech Pathologist VW1-VW4 (90,000 - 104,000 per annum, pro rata for part time).

Duties include

• Provision of high quality, evidence-based, person-centred care, including the application of case management principles and practice frameworks.
• Collaborative care planning with clients, carers and/or families in the context of their health, functional, psychosocial, cultural, environmental and economic needs. 
• Establish sound, collaborative working relationships and communicate effectively and professionally with a wide range of team members, TCP colleagues, key stakeholders and external service providers; to achieve complex discharge planning
• Support quality and continuous improvement initiatives, team processes and service development activities

Your Contribution

• Recent experience working in an Aged Care or subacute setting
• Membership of, or eligibility for, relevant professional body
• Demonstrated ability to communicate clearly and work effectively in a multidisciplinary team
• Proven ability to work collaboratively with a range of stakeholders, including clients, families, external agencies and other health professionals
• Highly developed interpersonal skills that incorporate the qualities of excellent communication, negotiation, organisation, planning and problem solving
• Knowledge of and commitment to continuous quality improvement processes
• Superior written and verbal communication skills
• Demonstrated knowledge of the application of case management practice frameworks and principles, in the context of the current health care environment.
• An ability to supervise students and/or other staff members as required
• Evidence of commitment to professional development in relevant areas

Transition Care Program

The Transition Care Program (TCP) is jointly funded by the Commonwealth and State and Territories. St Vincent’s TCP is resourced to follow the client through the service system and avoid unnecessary extension of hospital admissions and/or premature admission to residential care.

The intent of the Transition Care Program is to provide short-term support and active management for older people at the interface of the acute/subacute and residential aged care sectors.

The target group for Transition Care services at St Vincent’s are:

• Older people exiting from hospital who, via participation in the program, achieve further improvements to their physical, cognitive and psychosocial functioning and an improved capacity for living independently; and/or
• Older people exiting from hospital for whom the focus will be on actively maintaining their functioning while assisting them and their family/carers make appropriate long term care arrangements.

The Transition Care Program is located at the St Georges Health Service in Kew, but services all of the St. Vincent’s Hospital Melbourne catchment.
