ABOUT THE ROLE
We are seeking an experienced and compassionate Registered Nurse to join our Salvos Home Care team until 31 January 2028. In this rewarding community-based role, you will provide clinical leadership, care coordination and nursing support to clients living independently in their own homes.
Working closely with Care Partners, Home Care Workers, families and external healthcare providers, you will play a key role in ensuring clients receive safe, person-centred care that promotes their health, wellbeing and independence.
KEY RESPONSIBILITIES
As a Registered Nurse, you will:
- Conduct comprehensive nursing assessments and develop, implement and review individualised care plans for clients living at home.
- Monitor changing clinical needs and coordinate timely interventions to support positive health outcomes.
- Liaise with GPs, allied health professionals, clients and families to ensure integrated and responsive care.
- Maintain accurate clinical documentation, care plans, incident reporting and risk assessments in line with organisational and regulatory requirements.
- Provide clinical guidance and support to Home Care Workers, promoting safe and effective care practices.
- Participate in case conferences, quality improvement initiatives and continuous professional development activities.
ABOUT YOU
To be successful, you will have:
- Current unrestricted AHPRA registration as a Registered Nurse.
- Minimum two years' post-graduate nursing experience, ideally within aged care, community nursing or home care services.
- Strong clinical assessment and care planning skills.
- Excellent communication and relationship-building abilities.
- Sound understanding of Aged Care Quality Standards and person-centred care principles.
- A current driver's licence and willingness to travel throughout the local community.
