Position Summary
We are seeking a dedicated Physical Therapist (PT) to join our in-house rehabilitation team. In this role, you will evaluate residents, develop individualized treatment plans, and provide skilled therapy services designed to improve mobility, strength, function, and independence.
Key Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide skilled physical therapy services in accordance with physician orders
- Monitor resident progress and adjust treatment plans as needed
- Collaborate with physicians, nursing staff, families, and interdisciplinary team members
- Complete accurate and timely documentation
- Participate in care plan meetings and discharge planning
- Educate residents and caregivers on exercises, safety, and mobility strategies
- Maintain compliance with all regulatory and professional standards
- Support positive resident outcomes and quality-of-life goals
Qualifications
- Available Sunday through Thursday (weekend availability required)
- Degree from an accredited Physical Therapy program
- Current South Carolina Physical Therapist license in good standing
- Long-term care or skilled nursing experience preferred
- Experience with PointClickCare (PCC) and Casamba preferred
- Strong clinical, communication, and organizational skills
- Must maintain all required continuing education and licensure requirements
