What You Will Do

As the Gynecology and Hematology Coordination Nurse, you play a key role in coordinating and supporting patient care for gynecology and hematology services, with a primary focus on the IHI Women's Specialty Clinic. In this role, you will provide clinical support during onsite clinic visits, ensuring medication orders, care plans, and follow-up needs are addressed in a timely manner. Responsibilities also include telephone triage, patient education, care coordination, and the development and implementation of individualized care plans.

The Opportunity

  • Provide nursing support during onsite IHI Women's Specialty Clinic appointments, including patient intake, vital signs, medication reconciliation, procedure assistance, and patient education before, during, and after visits. Support many of the pre-and post-visit responsibilities for weekly telehealth visits.
  • Coordinate any pre-visit requirements, including obtaining consents, providing patient education, and facilitating pre-medication needs.
  • Triage incoming patient calls with a focus on women's health services and provide appropriate follow-up and after-hours care support.
  • Assess patient needs and communication nursing findings, recommendations, and care concerns promptly to providers, care team members, patients, and families in accordance with established triage protocols and clinical communication standards.
  • Serve as patient advocate by supporting patients and families, addressing barriers to care, and monitoring healthcare outcomes.
  • Collaborate effectively within a multidisciplinary care team and actively participate in the organization's patient-centered clinical care model.
  • Under the direction of a physician, educate patients on medications, treatment plan, and when to seek emergent care, while monitoring and reporting patient responses to treatment.
  • Communicate laboratory, diagnostic, and imaging results to patients and families in a clear, compassionate, and timely manner.
  • Manage medication prescriptions and refills under provider direction, including factor replacement therapies as applicable.
  • Utilize the electronic health record (EHR) to accurately review, document, and manage patient information while maintaining complete and compliant medical records.
  • Support ongoing initiatives, goals, and growth of the IHI Women's Specialty Clinic program through active participation in program development and quality improvement efforts.

Performance Requirements

Knowledge

  1. Knowledge of nursing assessment, clinical decision-making, and telephone triage protocols, including medical terminology, anatomy, and physiology.
  2. Knowledge of healthcare systems, clinical workflows, care coordination processes, and available community resources.
  3. Knowledge of basic laboratory procedures, diagnostic testing, and healthcare technology, equipment, and supplies used in patient care.
  4. Knowledge of medical record documentation standards, confidentiality requirements, and applicable federal and state regulations related to the release of health information.
  5. Knowledge of patient education principles and the development of home care instructions to support safe and effective self-management when immediate medical attention is not required.

Skills

  1. Strong critical thinking, clinical judgment, and prioritization skills in a fast-paced healthcare environment.
  2. Excellent time management, organization, problem-solving, and care coordination skills, with the ability to manage multiple priorities simultaneously.
  3. Proficiency in reviewing, interpreting, and documenting patient information within electronic health record (EHR) systems to support informed clinical decision-making.
  4. Strong interpersonal and communication skills, including the ability to navigate sensitive or complex patient and family interactions with professionalism and empathy.
  5. Ability to assess, triage, and respond appropriately to urgent and emergent situations while maintaining composure and sound clinical judgment.
  6. Ability to accurately interpret and follow written and verbal clinical orders.

Abilities

  1. Ability to engage patients and families as active participants in the development, implementation, and ongoing management of individualized care plans.
  2. Ability to communicate clearly, calmly, and compassionately with patients, families, providers, and interdisciplinary team members.
  3. Ability to gather, evaluate, and synthesize relevant clinical information to support timely and appropriate patient assessments and interventions.
  4. Ability to work independently while exercising sound judgment and recognizing when collaboration or escalation is necessary.
  5. Ability to document accurately, professionally, and thoroughly in the medical record, ensuring clear communication of patient status, interventions, and care plans.
  6. Ability to safely perform clinical treatments and procedures and accurately calculate medication dosages and related mathematical computations.
  7. Ability to adapt to changing patient needs, clinical priorities, and healthcare environments while maintaining a high standard of patient care.

Requirements

  • Graduation from an approved nursing program.
  • Valid RN nursing license, able to practice in the state of Indiana.
  • Minimum of 2 years of related nursing experience; preferably in collaborative chronic disease patient care, hematology, or gynecology.
  • Current BLS certification.
  • All IHI employees are expected to enable multi-factor authentication via their personal smart phone/smart device in order to access IHI systems as a requirement of the role.  

This position is open to residents of the State of Indiana, and requires routine presence at our Center in Indianapolis, IN. Frequency of on-site presence is dependent upon role and department needs.
