About the role

We're looking for a passionate and proactive Case Manager to join our Homeless Outreach Program in Nunawading, VIC.

As the first point of connection for many people experiencing homelessness, you'll engage with individuals who are sleeping rough or living in insecure accommodation, helping them access housing, navigate support services, and achieve sustainable outcomes.

Working in a dynamic outreach environment, you'll build strong relationships, advocate for client needs, and deliver person-centred support that empowers people to regain stability and independence.

Reporting to the Team Leader, this is a permanent full-time position based onsite in Nunawading, VIC.

Conditions

Salary and conditions are aligned with the SCHADS Award Level 5.

How you will make an impact

  • Provide holistic, strengths-based case management, including assessments, support planning, review and case closure
  • Engage with people sleeping rough and those at risk of homelessness, supporting them to access safe and sustainable housing outcomes
  • Build meaningful, trusting relationships that empower individuals to achieve their goals
  • Advocate for clients and connect them with housing, health, financial and community support services
  • Work collaboratively with external agencies and stakeholders to coordinate effective support pathways
  • Contribute to a culture of inclusion, compassion and client-centred practice

What you will bring

  • Relevant tertiary qualifications in Social Work, Human Services, Community Services, Disability or a related field
  • Demonstrated experience in case management, homelessness services, community services, housing support, outreach, or a related sector
  • A strong understanding of homelessness, housing pathways, trauma-informed practice and client-centred support models
  • Experience working directly with vulnerable and diverse communities within a front line, client-facing environment
  • Exceptional relationship-building, engagement and advocacy skills
  • Strong written communication skills and confidence using client management and case recording systems
  • Proven ability to work collaboratively with internal and external stakeholders to achieve positive client outcomes
  • Commitment to culturally safe, inclusive and strengths-based practice
  • Current and valid Victorian Driver's Licence
  • Current Victorian Employee Working with Children Check
