Key Responsibilities

Client Support Executives obtain referrals for services as well as promote, educate, and market all company services. Serving as a liaison between Optimal Care and referral sources you will coordinate care for referred patients from home health, non-medical home care, hospitals, and other medical community partners. This position aligns closely with industry-standard roles including titles such as Home Health Sales Specialist, Home Health Care Liaison, Home Health Business Development Coordinator, or Account Executive, Home Heath Services. In this role you will be responsible for:

• Drive Sales by building relationships with healthcare providers and community partners to increase Home Health referrals.

Key Responsibilities

The Physical Therapy Assistant administers physical therapy to patients on an intermittent basis in their place  of residence. This is performed in accordance with physician or advanced practice practitioner’s  orders and plan of care under the direction and supervision of the Home Health leadership team.

Key Responsibilities 

The Occupational Therapist administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician or advanced practice practitioner’s orders and plan of care under the direction and supervision of the Home Health leadership team.

Work Environment and Schedule

This position is based in our office with minimal travel for marketing activities, public relations, and occasional patient home visits. The role requires high flexibility - you must be available during all operating hours and responsive to urgent clinical and operational needs.  Frequent disruptions to routine are expected as you respond to patient emergencies, staff questions, referral source requests, and operational challenges. Key Responsibilities 

A Speech Pathologist (SLP) administers speech therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Administrator/Clinical Manager. Speech therapy services are furnished only by or under the supervision of a qualified speech pathologist or audiologist. 

In this role you will be responsible for: 

• Provides a full range Speech Language Pathology Services as ordered by physician
• Periodically participates with all other home care personnel in patient care planning 
• Takes initial history and makes initial evaluation, performs all skilled procedure 
• Consults with physicians regarding change of treatment, writes reports to physicians regarding patient’s progress 
• An initial evaluation, including a plan of treatment and goals, must be completed and submitted to the physician for approval
• A recertification by a physician every 60 days is necessary if further treatment is to be continued
• A progress note is written each visit
• A reevaluation is written when the expected duration of treatment is reached
• A summary is written upon patient’s discharge

Required Qualifications 

• A Master’s or Doctoral degree in Speech Language Pathology and is licensed as a Speech Language Pathologist by the state where they furnish service 
• Knowledge of medical terminology
• Reliable transportation and valid automobile insurance coverage 

Desired Qualifications 

• Two (2) years’ experience as a Speech Therapist preferred. Home Health experience preferred
