Our homes serve as the foundation for our loved ones, our communities, and our health. It comes as no surprise that most older adults wish to remain in the comfort of their homes as they age. However, many find it increasingly challenging to do so, driving demand for an integrated solution that supports older adults and their loved ones when they need it most.
Habitat Health plans to meet this need through the Program of All-Inclusive Care for the Elderly (PACE) ‒ a joint Medicare and Medicaid program with roots in California since the 1970s ‒ by providing older adults with care that empowers them to remain in their communities.
