This role is an integral, in-office member of our clinical care team, working alongside fellow Patient Care Coordinators and collaborating with Ennoble Care providers within a family-practice–style model of care.
While not patient-facing, the position plays a critical role in supporting patient-centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings.
