Ennoble Care is seeking a full-time Patient Care Coordinator to join our team in an office-based setting in Gulf Breeze, Florida. This role is an integral, in-office member of our clinical care team, working alongside fellow Patient Care Coordinators and collaborating with Ennoble Care providers within a family-practice–style model of care.
While not patient-facing, the position plays a critical role in supporting patient-centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings.

KEY RESPONSIBILITIES
- Coordinate patient-centered care activities using Ennoble Care’s electronic medical record (EMR) system
- Support Ennoble Care providers and clinical teams through accurate documentation, follow-up tracking, and care coordination
- Serve as a key point of communication between providers and internal care teams regarding clinical updates, patient needs, and care coordination priorities
- Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate manner
- Assist with care transitions, referrals, and coordination with external partners, as needed
- Collaborate closely with fellow Patient Care Coordinators and internal care team members within an office-based environment
- Maintain accurate, timely documentation in accordance with established workflows, documentation standards, and escalation protocols to support timely decision-making and efficient clinical operations
- Contribute positively to team collaboration, workflow efficiency, and day-to-day office operations
REQUIREMENTS
- Legally authorized to work in the United States of America
- Active, unrestricted clinical certification or licensure, such as:
- Patient Care Technician (PCT)
- Certified Nursing Assistant (CNA)
- Medical Assistant (MA) / Certified Medical Assistant (CMA)
- Licensed Practical Nurse (LPN)
- Registered Nurse (RN)
- Or comparable credential
- Minimum 1 year of experience in a clinical and administrative or patient care coordination role within a healthcare setting
- Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and internal systems
- Cultivate and maintain positive, approachable working relationships while demonstrating a consistent, dependable presence
- Well-developed organizational, time-management, and attention-to-detail skills, paired with an optimistic approach and initiative-driven mindset
- Strong working knowledge of healthcare terminology and care coordination principles
- Proficiency with Microsoft 365, including Outlook, Word, and Excel
- Ability to work full-time, Monday through Friday, 7:00 a.m. to 3:30 p.m.CST, in an office-based setting in the Gulf Breeze, Florida area
- No weekends
- No on-call
- Daytime hours
- Seven (7) company-paid holidays annually

Job Description:
Ennoble Care is looking for a Full-time, Care Coordinator, Inbound (Call Center Representative) that will work out of our Marlton, New Jersey office. This position is responsible for ensuring Ennoble Care is providing high quality care services. They work with clinicians, staff and patients to reach healthcare goals and keep the lines of communication open. As a Care Coordinator, Inbound (Call Center Representative), you should be compassionate, experienced, and highly organized. In this role, you will ensure that our patients immediate needs are taken care of.
Key Responsibilities:
- Frequent contact with patients to provide support, and manage compliance with the care management programs to increase positive outcomes
- Document all client communications (verbal or written) accurately
- Communication from Primary Care Clinician or designee regarding patient emergent needs and/or life-threatening episodes and to ensure comprehensive care plans are complete and accurate
- Demonstrate ability to work with various cross-organizational areas to meet the needs of Ennoble Care’s patients, their family members, and partner facilities
- Become skilled at using technology including secure email, telephone system, electronic medical records, etc.
- Adherence to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communications
- Excellent customer service skills demonstrated by positive feedback from customers and patients
- Contribute as a positive member of the department by supporting all members of the team in a productive and constructive manner
Qualifications:
- Must be comfortable with speaking on the phone for large amounts of the day
- Must be compassionate and empathetic towards our patients, always demonstrating exceptional customer service
- Ability to take accurate notes to document each task in a timely manner
- Ability to multitask between different patients and workstreams while remaining organized and efficient with time
- Ability to thrive in a fast-paced environment
- Must be proficient in using a computer, including Outlook and other Microsoft Office programs
- Candidate must be able to pass a drug screen, background check, have a positive attitude, adapt positively to change, be a team player, and be willing to learn new skills on a continuous basis.
- Nice to have: Knowledge of basic healthcare terms, conditions, roles, and basic care principles is helpful

To support a dedicated group of patients. The work is very much family-practice focused, including a strong emphasis on remote patient monitoring (RPM) and proactive care coordination.
REQUIREMENTS
- Legally authorized to work in the United States of America
- Active, unrestricted clinical certification or licensure, such as:

REQUIREMENTS

- Or comparable credential
- Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
- Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person
- Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting
- Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset
- Strong working knowledge of healthcare terminology and care coordination principles
- Proficiency with Microsoft 365, including Outlook, Word, and Excel
- Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Reston, Virginia area, with a primary focus on the Fairfax vicinity.

Job Description:
Ennoble Care is looking for a full-time, experienced Patient Care Coordinator that will work out of our Hackensack, NJ office, who aligns with our motto, "To Care Is An Honor". This position is responsible for ensuring Ennoble Care is providing high quality care services. They work with clinicians, staff and patients to reach healthcare goals and keep the lines of communication open. As a Patient Care Coordinator you should be compassionate, experienced, and highly organized. In this role, you will play an important part in our ability to provide exceptional care by managing the individual care providers, including scheduling and providing support for the caregivers and families.
Responsibilities:
- Complete individualized patient care plans and perform care management and care coordination services using Ennoble Care’s electronic medical record system
- Frequent contact with patients to provide care coordination, support, and manage compliance with the care management programs to increase positive outcomes
- Document all client communications (verbal or written) accurately
- Communication to and from Primary Care Clinician or designee regarding patient emergent needs and/or life-threatening episodes and to ensure comprehensive care plans are complete and accurate
- Keep Team Supervisor informed of all issues pertinent to the care plan process and any known or perceived issues
- Demonstrate ability to work with various cross-organizational areas to meet the needs of Ennoble Care’s patients, their family members, and partner facilities
- Become skilled at using technology including secure email, telephone system, electronic medical records, etc.
- Adherence to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communications
- Excellent customer service skills demonstrated by positive feedback from customers and patients
- Contribute as a positive member of the department by supporting all members of the team in a productive and constructive manner
Equipment Operation:
- Utilization of a computer, telephone, copy machine, and other office equipment as necessary
Qualifications:
- Must be comfortable with speaking on the phone for large amounts of the day
- Must be compassionate and empathetic towards our patients, always demonstrating exceptional customer service
- Ability to take accurate notes to document each task in a timely manner
- Ability to multitask between different patients and workstreams while remaining organized and efficient with time
- Ability to thrive in a fast-paced environment
- Must be able to work full-time, Monday through Friday, daytime hours, on-site in Hackensack, NJ.
- Must be proficient in using a computer, including Outlook and other Microsoft Office programs
- Knowledge of basic healthcare terms, conditions, roles, and basic care principles

Job Summary:
Ennoble Care is looking for a Full-time, Patient Care Coordinator, Empassion to work out of our Marlton, New Jersey office. This position is responsible for ensuring Ennoble Care is providing high quality care services. They work with clinicians, staff and patients to reach healthcare goals and keep the lines of communication open. As a Patient Care Coordinator, Empassion you should be compassionate, experienced with patient care, and highly organized. In this role, you will play an important part in our ability to provide exceptional care by managing the individual care providers, including scheduling and providing support for the caregivers and families.
PLEASE NOTE: THIS IS A FULL-TIME, IN-OFFICE POSITION.
Responsibilities:
- Complete individualized patient care plans and perform care management and care coordination services using Ennoble Care’s electronic medical record system
- Frequent contact with patients and families to provide care coordination, support, and manage compliance with the palliative care program to increase positive outcomes
- Conduct and document simple health, psychological and social needs assessments for chronically ill patients, and effectively communicate recommendations to the Chronic Care Clinician
- Document all client communications (verbal or written) accurately
- Communication to and from Chronic Care Clinician or designee regarding patient emergent needs and/or life-threatening episodes and to ensure comprehensive care plans are complete and accurate
- Keep Team Supervisor informed of all issues pertinent to the care plan process and any known or perceived issues
- Demonstrate ability to work with various cross-organizational areas to meet the needs of Ennoble Care’s patients, their family members, and partner facilities
- Become skilled at using technology including secure email, telephone system, electronic medical records, etc.
- Adherence to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communications
- Guide sensitive conversations with patients and families regarding goals of care, advance care planning and the transition to hospice
- Contribute as a positive member of the department by supporting all members of the team in a productive and constructive manner
Equipment Operation:
- Utilization of a computer, telephone, copy machine, and other office equipment as necessary
Qualifications:
- Must be comfortable with speaking on the phone for large amounts of the day
- Must be compassionate and empathetic towards our patients, always demonstrating exceptional customer service
- Must be comfortable and skilled at talking with patients and families about end-of-life care and the transition to hospice care
- Must have knowledge of basic healthcare terms, conditions, roles, and basic care principles
- Prior experience working in healthcare (e.g., nurses aid, medical assistant or community health worker in chronic care management or hospice) is highly preferred
- Ability to take accurate notes to document each task in a timely manner.
- Ability to multitask between different patients and workstreams while remaining organized and efficient with time.
- Ability to thrive in a fast-paced environment.
- Must be able to work from full-time, Monday through Friday, on-site, in an office.
- Must be proficient in using a computer, including Outlook and other Microsoft Office programs.

This role provides care coordination and support services, with some in-person collaboration as needed. Job Summary:
The Memory Care Coordinator is responsible for care coordination, caregiver support, and dementia education, ensuring that patients and their care partners receive the guidance they need. This position involves engagement with care partners and healthcare teams, supporting them through education, resource navigation, and ongoing case management. The ideal candidate will have hands-on experience in dementia care, with a deep understanding of the challenges faced by both patients and their care partners. Effective communication, empathy, and the ability to offer emotional support are essential to success in this role While prior experience in dementia care is valuable, we are willing to train the right candidate with strong communication skills, empathy, and a passion for caregiving support.
Responsibilities:
Care Coordination & Care Partner Support
- Act as a remote point of contact for care partners, guiding them through dementia-related challenges and connecting them to appropriate resources.
- Develop individualized care plans, conduct assessments, and collaborate with healthcare providers to ensure comprehensive, person-centered support.
- Provide education and emotional support to care partners, equipping them with strategies to manage dementia-related behaviors and navigate care decisions.
- Identify and recommend community resources such as respite care, support groups, and memory care programs.
Virtual Case Management & Communication
- Maintain regular phone, video, and digital communication with care partners, ensuring ongoing support and intervention as needed.
- Maintain open communication with the Memory Care Program Manager, regularly meeting to discuss patient care, care partner goals, and interventions. Provide updates on participant progress, engage in case discussions, and coordinate necessary adjustments to care plans to ensure efficient support and alignment with the care process. Foster collaborative communication with internal and external teams to meet patient and care partner needs.
- Document all interactions, assessments, and recommendations within electronic medical record (EMR) systems for accurate record-keeping and compliance.
Care Partner Advocacy & Resource Navigation
- Provide empathetic, responsive, and professional service in all interactions with patients, care partners, and colleagues. Demonstrate compassion and empathy in all communication, ensuring that patients and their care partners receive the highest level of support.
- Serve as an advocate for care partners, helping them access services, understand dementia progression, and make informed decisions.
- Offer virtual educational sessions and caregiver coaching, supporting families in their caregiving journey.
- Coordinate with internal and external teams to ensure care partners receive seamless, efficient support.
Technology Utilization & Documentation
- Utilize telehealth platforms, secure messaging systems, and internal databases to streamline communication and case management.
- Maintain accurate, timely documentation in compliance with company policies and regulatory requirements.
Team Collaboration & Professional Development
- Work closely with healthcare providers, memory care specialists, and program leadership to refine care coordination strategies.
- Actively participate in virtual team meetings and in-person training sessions as required.

Qualifications:
Experience & Knowledge
- Experience working with individuals diagnosed with dementia or other memory-related conditions, preferably in a senior or geriatric care setting. Personal experience as a care partner for individuals with dementia is also highly valued. A foundational understanding of dementia and memory-related conditions is preferred.
- Familiarity with basic healthcare terminology, conditions, and care principles to effectively understand patient needs and care plans.
- Knowledge of local resources for dementia patients and care partners, such as respite care, support groups, and medical specialists.
Communication & Technology
- Comfortable with frequent phone, video, and digital communication to engage care partners and healthcare teams.
- Strong verbal and written communication skills to provide clear, compassionate guidance.

Ennoble Care is seeking a full-time Care Coordinator, Embedded @ Partnered-Dialysis Clinics in the Tulsa, Oklahoma area. This on-site role serves as a key liaison within a partnered dialysis facility, ensuring coordinated, patient-centered care through structured coordination, strong clinical collaboration, and proactive communication.
This Care Coordinator works directly within one of our partnered-dialysis clinics to engage patients in person, coordinate care plans in collaboration with Ennoble providers and facility staff, schedule and organize provider visits, and ensure timely follow-up and clear communication with patients, families, and the clinical team.
