Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Reston, Virginia area, with a primary focus on the Manassas vicinity. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.

KEY RESPONSIBILITIES
- Coordinate care for long-term care residents using Ennoble Care’s EMR
- Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions
- Collaborate closely with Ennoble Care providers, within the long-term care setting
- Serve as a key liaison between providers, facility staff, residents, and families
- Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner
- Accurately document patient interactions and care coordination activities
- Follow established workflows, documentation standards, and escalation protocols
- Contribute positively to team collaboration and strong facility partnerships
WHAT 'EMBEDDED' MEANS AT ENNOBLE CARE
This is not a remote or call-center role.
As an Embedded Care Coordinator, you are physically on-site in one or more partnered long-term care facilities, working alongside providers and facility staff. You build consistent relationships with residents and families and become a trusted, familiar presence within the facility -- supporting care the way a traditional family practice would, but in the long-term care setting. To support a dedicated group of patients. The work is very much family-practice focused, including a strong emphasis on remote patient monitoring (RPM) and proactive care coordination.
REQUIREMENTS
- Legally authorized to work in the United States of America
- Active, unrestricted clinical certification or licensure, such as:
- Patient Care Technician (PCT)
- Certified Nursing Assistant (CNA)
- Medical Assistant (MA) / Certified Medical Assistant (CMA)
- Licensed Practical Nurse (LPN)
- Registered Nurse (RN)
- Or comparable credential
- Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
- Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person
- Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting
- Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset
- Strong working knowledge of healthcare terminology and care coordination principles
- Proficiency with Microsoft 365, including Outlook, Word, and Excel
- Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Reston, Virginia area, with a primary focus on the Manassas vicinity.
- No weekends
- No on-call
- Daytime hours
- Seven (7) company-paid holidays annually
- Transportation criteria:
- Valid, unencumbered driver’s license
- Consistent, reliable transportation to fulfill the duties of this position

Job Summary:
Ennoble Care is looking for a Per Diem Chaplain to work within and around our Arlington / Loudoun / Fairfax / Prince William County, VA area. This role will be as needed as we establish and grow our new hospice operations in the area with the potential to eventually become full time as the need for a chaplain grows.
The Hospice Chaplain under the supervision of the Clinical Director works as an integral part of an interdisciplinary team and provides spiritual, emotional and some bereavement care to support hospice patients, clients, and families in facilities and in-home settings. Must possess knowledge of, and commitment to hospice philosophy of care.
Responsibilities:
- The chaplain provides pastoral/spiritual care to patients and/or families.
- Visits and ministers with patients. Works with the Clinical Director and other appropriate persons in developing a spiritual care continuum of care appropriate for hospice patients/families.
- Conducts spiritual assessments and develops a care plan that addresses a range of needs including physical, emotional and spiriual, outcomes and goals with a view to providing a comprehensive assessment of the patient.
- Provide pastoral support at bedside, funerals, memorials as requested and needed.
- Documents visits with patients per Company and compliance policy.
- Functions as a member of the interdisciplinary team.
- Organizes Company-sponsored memorial services as appropriate.
- The chaplain provides emotional/spiritual support to Staff.
- Provides spiritual/emotional support and encouragement to staff members as needed and appropriate.
- Participates in staff meetings, Interdisciplinary Team meetings and In-Services.
- The chaplain initiates and maintains communication with area inter-faith clergy.
- Facilitates spiritual care, acting as a bridge between patients, families and clergy to provide supportive end-of-life sacramental support and pastoral care as requested.
- Assists in educating staff about specific religious needs and issues of patients/families which affect their care and treatment.
- The chaplain performs other duties as assigned or requested.
- Reports to and is primarily accountable to the Clinical Director.
- Works collaboratively with the interdisciplinary team to ensure holistic patient care.
- The chaplain contributes value-added activities that result in positive well-being among clients, patients, families and staff.
Qualifications:
- Master’s of Divinity Degree or equivalent in an area of theological studies required.
- At least four units of Clinical Pastoral Education (CPE). Candidate must be Board Certified as a Chaplain or eligible for Board Certification under the Association for Clinical Pastoral Education (ACPE) or the College of Pastoral Supervision and Psychotherapy (CPSP).
- Additional qualifications include excellent communication and pastoral skills. Must have a high degree of emotional energy and mental alertness and ability to deal with the challenge of supporting patients and loved ones who experience pain, suffering, death and grief.
- Good organizational and decision-making skills.
- Computer Literacy and ability to perform extensive and timely charting timely using the Company’s EMR System.
Licensure:
- Certification or eligibility for certification through the CPSP or ACPE.
Experience:
- A minimum of two years of pastoral experience in a healthcare setting. Hospice experience is desirable, demonstrable empathy, compassion, sensitivity and ability to support patients/families of inter-faith traditions.
Professionalism/Growth:
- Strong interpersonal skills. Ability to accept and work with diverse ages, different lifestyles, cultures, socio-economic and religious backgrounds, spiritual beliefs and values.
- Professional knowledge and clinical ability sufficient to develop and maintain collegial and collaborate relationships with physicians, nurses, social workers, clients and other members of the healthcare team.
- Ability to work independently, take initiative and be held accountable. Adequate physical and emotional health is very important.
- Ecclesiastical endorsement or Ordination in a Religious Setting is desirable

The caregiver (CNA) will be responsible for:
- Care for census of 5-6 patients
- Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
- Provides assistance in toileting, positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.

Job Summary:
Ennoble Care is hiring for a Full-time Hospice Registered Nurse Case Manager to work within and around the Arlington / Fairfax / Loudoun County region in Virginia. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.
Responsibilities:
- Shall be assigned patient caseload by the Patient Care Coordinator or designee
- Assume primary responsibility for a patient caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process
- Initiate communication with attending physicians, Hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient and loved ones/caregivers
- Maintain regular communication with the attending physician concerning patient and loved ones/caregivers
- Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support
- Provide accurate and timely documentation in electronic medical records.
- Participates in and contributes appropriate information for Interdisciplinary Group meetings

- Meet regularly with the Hospice nursing staff to review problems or unique issues from caseloads; share professional support; and exchange feedback aimed toward enhancing professional growth
- Attend patient care conferences and facilitate discussion of issues from caseload for full staff discussion, consultation and evaluation
- Supervise and document the NAC every two weeks or as assigned
- Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues
- Assume the role of associate nurse when responding to patient/loved ones/caregivers of other primary nurses during off-hour times or as requested by the Patient Care Coordinator to fill patient/family needs during the absence of other primary nurses
- Provide appropriate support at time of death and period of bereavement
- Participate in the Hospice orientation and in-service training programs for professional staff
- Participate in agency and community programs as required to promote professional growth and understanding of hospice care
- Maintain familiarity with policies of the agency and rules and regulations of State and Federal bodies which govern hospice
- Accept other assignments as appropriate
Qualifications:
- Possession of a current license to practice as a registered nurse in the State of VA
- At least two years of work experience as a professional nurse preferably at least one of the years in hospice care
- Ability to assess and respond to the needs of patients and loved ones/caregivers in varied settings.
- Skilled in nursing practice, able to cope with loved ones/caregivers’ emotional stress and tolerant of individual lifestyles
- Ability to make meaningful observations, skilled assessments, and write comprehensive, accurate reports
- Must be a team player who supports an interdisciplinary team concept of caring for patient and loved ones/caregivers
- Must have a valid driver’s license, a good driving record, the use of a reliable vehicle, and the ability to drive year-round to locations
- Must be able to take direction, work independently and effectively as part of a multidisciplinary team in a positive, professional manner which supports the mission of the organization
Starting Pay: $90,000/yr

About the Role:
Ennoble Care is looking for a Full-time Hospice On-Call Registered Nurse to provide care and support to terminally ill patients within and around the Arlington / Fairfax / Loudoun County, VA area. Your presence and services should bring comfort to the person who faces the end of their lives and help them reach acceptance and peace of mind. A compassionate nature and excellent interpersonal skills will make you a good candidate for this position. We expect you to be knowledgeable in caring for the terminally ill, performing tasks such as administering medication and providing emotional support. The goal is to help patients live their final days with dignity and comfort. The responsibilities for this position include the following:
- Collaborate with physicians and other professionals to plan care
- Triage phone calls
- Make visits to the patient as needed to evaluate their condition
- Provide support and comfort according to individual patient’s needs
- Work with caregivers to plan and execute excellent patient care
- Help in administering medication
- Alleviate sudden symptoms of the disease’s end phase
- Notify a doctor for a patient’s condition when deemed necessary
- Assist patients and their families with paperwork
- Maintain accurate records
- Help relatives cope with the upcoming loss and provide post-mortem support
About You:
- A valid RN license to practice within the State of VA
- Proven experience as hospice nurse or registered nurse
- Knowledge of nursing care processes and medication guidelines/side-effects
- Committed to safety precautions and high ethical standards
- Sound judgement and attention to detail
- Excellent communication skills and bilingual a plus

Pay Range: Starting at $80,500 salary

Position Summary
The Clinical Liaison serves as the first point of contact for hospitalized patients and their families who may benefit from Ennoble Care’s hospice and palliative care services. This role is responsible for educating patients, families, and healthcare partners about Ennoble Care’s programs, supporting informed decision-making, and facilitating smooth transitions of care. The Clinical Liaison collaborates closely with physicians, interdisciplinary team members, and acute care partners to develop and initiate individualized, goal-aligned care plans.
Key Responsibilities
- Serve as the primary liaison between Ennoble Care and acute care facilities, identifying patients appropriate for hospice or palliative care services.
- Introduce and educate patients and families on Ennoble Care’s hospice and palliative care philosophy, services, and benefits.
- Provide emotional support and guidance to patients and families during care transitions.
- Collaborate with patients, families, physicians, and the interdisciplinary team to develop care plans aligned with patient goals and wishes.
- Conduct nursing assessments and make clinical judgments based on patient needs and eligibility criteria.
- Initiate care plans and coordinate discharge planning in collaboration with Ennoble Care’s hospice and palliative care medical consultants.
- Communicate patient goals, clinical status, and treatment plans effectively with interdisciplinary team members.
- Promote Ennoble Care’s hospice and palliative care programs within hospitals, skilled nursing facilities, and other referral sources.
- Build and maintain strong relationships with facility leadership and staff through education, in-service trainings, and community outreach.
- Rotate to other regions or departments as business needs require.
Qualifications & Requirements
- Minimum of one (1) year of experience in a comparable clinical setting or unit.
- Minimum of one (1) year hospice and/or marketer experience.
- Needs hospice and/or palliative care experience.
- Graduate of an accredited nursing program as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) required.
- BSN preferred for RN candidates.
- Current nursing licensure as an RN or LPN in the applicable state(s) of practice (compact state licensure accepted where applicable).
- Valid driver’s license in good standing with access to a reliable vehicle during working hours.
Base Salary: $50,000 - $95,000, Plus: Monthly, Quarterly, and Annual Bonuses
