Job Summary:

Ennoble Care is seeking an experienced Full-time Social Worker Care Manager to service patients for our house calls department within and around the Toms River, NJ region. This position will focus on patients with mental health and substance abuse matters, and will based out of the Freehold, NJ office with travel outside of the office to patients home or facilities for appointments.

Responsibilities:

• Support the mental and physical health care of patients on an assigned patient caseload.
• Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.
• Provide patient education about common mental health and substance abuse disorders and the available treatment options.
• Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
• Support psychotropic medication management by consulting with the behavioral health consultant and then communicating with the primary care provider assigned.
• Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
• Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.
• Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
• Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR.
• Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.
• Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
• Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
• Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
• Be comfortable utilizing Excel and Word and completing assessments and productivity trackers daily.
• Time-management skills are a must and the BHCM needs to be able to manage their day effectively through calendars and the EMR system.
• This role is telephonic, however in person visits are done and the BHCM has to be comfortable completing them both ways.

Qualifications:

• The Social Worker Care Manager will have a minimum of MSW
• Preferred Licensure or provisionally licensed
• Behavioral Health experience and knowledge
• Must be licensed driver with an insured automobile in good working order
• Ability to communicate effectively, both verbally and in writing.
• Time Management skills

Starting Salary: $55,000/yr

Ennoble Care is looking for a full-time, experienced Patient Care Coordinator that will work out of our Hackensack, NJ office, who aligns with our motto, "To Care Is An Honor". This position is responsible for ensuring Ennoble Care is providing high quality care services. They work with clinicians, staff and patients to reach healthcare goals and keep the lines of communication open. As a Patient Care Coordinator you should be compassionate, experienced, and highly organized. In this role, you will play an important part in our ability to provide exceptional care by managing the individual care providers, including scheduling and providing support for the caregivers and families.

Responsibilities:

• Complete individualized patient care plans and perform care management and care coordination services using Ennoble Care’s electronic medical record system
• Frequent contact with patients to provide care coordination, support, and manage compliance with the care management programs to increase positive outcomes
• Document all client communications (verbal or written) accurately
• Communication to and from Primary Care Clinician or designee regarding patient emergent needs and/or life-threatening episodes and to ensure comprehensive care plans are complete and accurate
• Keep Team Supervisor informed of all issues pertinent to the care plan process and any known or perceived issues
• Demonstrate ability to work with various cross-organizational areas to meet the needs of Ennoble Care’s patients, their family members, and partner facilities
• Become skilled at using technology including secure email, telephone system, electronic medical records, etc.
• Adherence to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communications
• Excellent customer service skills demonstrated by positive feedback from customers and patients
• Contribute as a positive member of the department by supporting all members of the team in a productive and constructive manner

Equipment Operation:

• Utilization of a computer, telephone, copy machine, and other office equipment as necessary

Qualifications:

• Must be comfortable with speaking on the phone for large amounts of the day
• Must be compassionate and empathetic towards our patients, always demonstrating exceptional customer service
• Ability to take accurate notes to document each task in a timely manner
• Ability to multitask between different patients and workstreams while remaining organized and efficient with time
• Ability to thrive in a fast-paced environment
• Must be able to work full-time, Monday through Friday, daytime hours, on-site in Hackensack, NJ.
• Must be proficient in using a computer, including Outlook and other Microsoft Office programs
• Knowledge of basic healthcare terms, conditions, roles, and basic care principles 
• Candidate must be able to pass a drug screen, background check, have a positive attitude, adapt positively to change, be a team player, and be willing to learn new skills on a continuous basis

PAY RATE- $18 an hour
