Liquid Personnel is recruiting Social Worker for its client's Locality Team at Herefordshire.

This is an excellent opportunity for a dedicated practitioner to work with a diverse range of adults, including older people, individuals with physical disabilities, learning disabilities, and mental health needs, delivering strengths-based and person-centred social work interventions that promote independence, wellbeing, and positive outcomes.

The successful candidate will manage a varied and complex caseload, undertaking statutory assessments, support planning, safeguarding enquiries, and risk management interventions. Working collaboratively with service users, carers, families, health professionals, and partner agencies, you will play a key role in ensuring adults receive the right support at the right time whilst maintaining choice, control, and dignity.

What will your responsibilities be?

• Managing a varied caseload involving older adults, individuals with physical disabilities, learning disabilities, and mental health needs.
• Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning to identify eligible needs and promote positive outcomes.
• Developing, implementing, and reviewing person-centred Care and Support Plans that maximise independence, wellbeing, and choice.
• Undertaking Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
• Leading and contributing to Section 42 safeguarding enquiries in line with Adult Safeguarding legislation and procedures.
• Assessing and managing complex risks, balancing individual choice with safety and positive risk-taking.
• Supporting Continuing Healthcare (CHC) assessments and processes where appropriate.
• Undertaking Section 117 aftercare responsibilities and reviews for eligible individuals.
• Working collaboratively with health professionals, community services, care providers, families, carers, and partner agencies to achieve positive outcomes.
• Preparing professional reports, assessments, and documentation to support decision-making and legal processes where required.
• Promoting strengths-based practice and empowering individuals to access community resources, support networks, and preventative services.
• Participating in multidisciplinary meetings, safeguarding discussions, reviews, and case conferences.
• Maintaining accurate, timely, and legally compliant records, assessments, reports, and support plans.

Qualifications & Experience:

To be successful in this role you must have, 

• Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
• Registration: Must be registered with Social Work England.
• Experience: Significant post-qualified experience within Adult Social Care services.
• Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, Continuing Healthcare processes, and associated statutory guidance.
• Assessment Skills: Proven ability to undertake complex assessments, reviews, support planning, and risk management interventions.
• Safeguarding Experience: Experience undertaking Section 42 enquiries and managing complex safeguarding concerns.
• Multi-Agency Working: Strong experience working collaboratively with health professionals, community services, care providers, families, and partner agencies.
• Communication Skills: Excellent communication, report-writing, analytical, negotiation, and decision-making skills.
• Professional Development: Interest in developing specialist practice through qualifications such as Practice Educator, Best Interest Assessor (BIA), and Approved Mental Health Professional (AMHP).
• Driving Licence: Full UK driving licence and access to a vehicle is essential.
• Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
