Experienced Social Workers needed for out client to support a Hospital Services and Intermediate Care Service at Salford Royal.
These post are office based due to the nature of the business - interviews will also be face to face
Working pattern Monday to Friday 08:00 to 16:00
3 years reference, gaps in employment history checked, any relevant quals as per JDPS
What will your responsibilities be?
- Undertake comprehensive assessments of patients’ social care, safeguarding, housing, and support needs to facilitate safe and effective discharge from hospital.
- Develop and coordinate person-centred discharge plans in partnership with patients, families, carers, and relevant professionals.
- Assess eligibility for social care services and identify appropriate packages of care, support, and community resources.
- Coordinate referrals and access to appropriate services, including domiciliary care, reablement, supported living, residential and nursing care.
- Work collaboratively within a multidisciplinary team to ensure timely and coordinated discharge planning.
- Advocate for patients’ rights, wishes, preferences, and wellbeing throughout the assessment and discharge process.
- Identify, assess, and respond appropriately to safeguarding concerns, escalating risks in accordance with relevant policies and procedures.
- Liaise with families, carers, health professionals, local authority services, and external agencies to ensure effective communication and continuity of care.
- Address housing and accommodation issues that may present barriers to safe discharge, making appropriate referrals where required.
- Maintain accurate, timely, and confidential records of assessments, decisions, interventions, and discharge arrangements in accordance with professional and organisational requirements.

Qualifications and Experience:
To be successful in this role you must have;
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
