Liquid Personnel is recruiting Social Worker for its client's Ageing Well Team at Wolverhampton.
This is an excellent opportunity for a dedicated practitioner to support older adults with a range of care and support needs, helping them maintain independence, improve wellbeing, and achieve positive outcomes through strengths-based and person-centred social work practice.
The successful candidate will work closely with older people, their families, carers, health professionals, and partner agencies to undertake assessments, support planning, safeguarding interventions, and reviews. You will play a key role in supporting individuals to remain independent within their own homes and communities whilst promoting choice, dignity, and quality of life.
This role requires a confident Social Worker with strong assessment skills, experience managing complex casework, and a sound understanding of legislation relating to older adults and Adult Social Care.
What will your responsibilities be?
- Managing a caseload of older adults with a range of care and support needs, including complex physical health conditions, frailty, dementia, and long-term illnesses.
- Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning to identify eligible needs and improve outcomes.
- Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and choice.
- Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
- Responding to safeguarding concerns and undertaking safeguarding enquiries in line with Adult Safeguarding legislation and local procedures.
- Assessing and managing risk appropriately whilst promoting positive risk-taking and individual independence.
- Supporting older adults through periods of crisis, hospital discharge, deterioration in health, bereavement, or changes in care needs.
- Working collaboratively with health professionals, community services, care providers, housing services, carers, families, and partner agencies.
- Coordinating care packages, community support services, equipment, and interventions to help individuals remain safely within their communities.
- Promoting access to community resources, preventative services, reablement opportunities, and support networks that enhance wellbeing and reduce isolation.
- Participating in multidisciplinary meetings, safeguarding discussions, reviews, and case conferences.
- Maintaining accurate, timely, and legally compliant records, assessments, reports, and support plans.
- Supporting carers through assessment, advice, guidance, and access to appropriate support services.
Qualifications & Experience:
To be successful in this role you must have,
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care services.
- Older Adults Experience: Experience supporting older adults with complex care and support needs within a community-based setting.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
- Assessment Skills: Proven ability to undertake Care Act assessments, reviews, support planning, and risk management interventions.
- Safeguarding Experience: Experience undertaking safeguarding enquiries and managing safeguarding concerns involving vulnerable adults.
- Multi-Agency Working: Strong experience working collaboratively with health professionals, community services, care providers, carers, families, and partner agencies.
- Communication Skills: Excellent communication, report-writing, analytical, negotiation, and decision-making skills.
- Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
- Organisational Skills: Ability to manage a complex caseload, prioritise competing demands, and work effectively within a busy Adult Social Care team.
