The Practitioner is responsible for the direct provision of intensive psychiatric rehabilitation services. The purpose of the REACH program is to assist individuals with serious and persistent mental illness and co-occurring substance use disorders in setting and achieving personal goals in their living, learning, working, and social environments. To this end, the scope of the Practitioner’s responsibilities includes establishing a working partnership with program participants, assistance with choosing roles, and settings in which people would like to live, learn, or work: identification of the skills and resources necessary for the chosen roles and settings; assistance with skill acquisition and effective implementation; and linkage with or creating the necessary supports/resources.

JOB DUTIES AND REQUIREMENTS:

• Maintain a direct service caseload and provide individualized IPR services including: (a)Establishment of effective and recovery-oriented working relationships; (b) Provision of individual and class on-site and community-based services; (c) Assistance with choosing desired roles in desired living, learning, working, and social environments; (d) Identification of the skills and resources necessary for the chosen roles and settings to be successful and satisfying; (e) Assistance with skill acquisition and effective skill implementation; and linkage with or creating of necessary support/resources.

• Complete assessments and service documentation on a timely basis and as required.

• Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information. 

• Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements. 

• Demonstrates/models the Network’s PCRAFT values during interactions with all customers.   

• Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.

• Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.

• Complies with Network and departmental policies regarding attendance and dress code. 

• Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.

• Other duties as assigned.

PHYSICAL AND SENSORY REQUIREMENTS:

Sit up to 2 hours per day; 1 hour at a time. Stand for up to 10 hours per day; 8 hours at a time. Walk 6 hours per day; 10 minutes at a time. Consistently lift, carry and push objects up to 10 lbs. Occasionally lift, carry and push objects up to 75 lbs. Transports patients weighing up to 500 lbs. via wheelchair, bed and/or stretcher. Frequently stoop and bend. Frequently reach above should level. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation and high and low frequencies. Must be able to see as it relates to general, near, far, color and peripheral vision.

EDUCATION:

A bachelor’s degree in behavioral health-related field, an associate's degree and 1 year of work experience in mental health direct service, a Certified Peer Specialist (CPS) certification and 1 additional year or volunteer work experience in mental health direct services, or a high school diploma or GED and 2 years' work experience in human services which must include 1 year of mental health direct service.

TRAINING AND EXPERIENCE:

Must have and maintain a valid driver’s license accepted by the Network’s insurance carrier and maintain a good driving record.
