ABOUT THE ROLE 

Daymark is partnering with leading healthcare providers across New England to bring supportive oncology care into the home for patients with cancer. As Daymark's Primary Care Physician, Supportive Oncology, you will be one of the clinical leads for that work and one our care teams turn to for guidance.

You will combine direct patient care with clinical oversight and be a visible leader in our provider partnerships. You will support each partnership clinically, enhancing how we work alongside a partner's primary care physicians and oncologists and making sure the care we deliver in the home is care that partners trust with their patients.

You will see patients virtually, partner with our Nurse Practitioners, help set the standard for how we document and communicate the care we deliver, and serve as a steward of Daymark's clinical program. You should be comfortable operating as a player/coach – rolling up your sleeves on direct care while shaping the processes that let partnerships thrive.

WHAT YOU'LL DO

• Provide direct patient care. You will care for patients across our Massachusetts and Rhode Island markets. You will perform comprehensive initial visits for newly enrolled patients, manage complex and high-acuity cases, support post-discharge transitions, and respond to clinical escalations from the care team. You will conduct assessments, place orders, prescribe medications, and document care in our EHR.

• Help build the care model. You will partner with Daymark's regional leadership to refine our SOPs, care pathways, and risk stratification approach, and contribute clinical expertise to the design of new programs and workflows as we grow.

• Represent Daymark clinically with our partners. You will serve as one of Daymark's clinical representatives to oncology practices, primary care physicians, health systems, accountable care organizations, and health plan partners in New England. You will bring clinical credibility to conversations with the practices whose patients we serve. Working alongside our partnerships and operations teams, you will help ensure Daymark meets the clinical and quality obligations we take on in each partnership, including quality gap closure for the patients in our care.

WHAT WE ARE LOOKING FOR

• Training and credentials
  • You are a board-certified physician (MD or DO) who completed residency training in internal medicine, family medicine, or geriatric medicine.
  • You are able and willing to enroll in Medicare under Daymark, and to complete that enrollment promptly after your start date.
  • You hold active, unrestricted medical licenses in both Massachusetts and Rhode Island.
  • You hold active DEA registration and are eligible to register in both states.
  • You are eligible to enroll as a Medicare provider and are not excluded, debarred, or sanctioned under any federal or state health care program.
• Experience
  • You have at least 5 years of practice experience including substantial direct patient care.
  • You have meaningful experience caring for patients with cancer, or in palliative care, hospice, or serious illness care. Board certification in Hospice and Palliative Medicine is strongly preferred.
  • You have supervised or collaborated with Nurse Practitioners and you're comfortable sharing accountability for the clinical performance of a team.
  • You have practiced in a value-based or shared savings environment – an ACO, a Medicare risk arrangement, or similar – and understand how quality measurement, care gap closure, and documentation affect a program's performance. Direct experience with home-based care is preferred.
• How you work
  • You are comfortable caring for patients and at ease meeting patients where they live. You are willing to travel as needed within Massachusetts and Rhode Island.
  • You are tech savvy and comfortable caring for patients both in person and virtually over video and web-based platforms.
  • You are comfortable with ambiguity and with building something that may not exist yet.
  • You communicate well with oncologists, primary care physicians, and the patients and families in between.

ADDITIONAL INFORMATION

$250,000 to $325,000 compensation, depending on experience.
