CenterWell in The Villages, a subsidiary of Humana Inc., is looking for a primary care physician for our Creekside Center in The Villages, FL. You will have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are affecting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a versatile approach.
As an Advanced Practice Provider, you will report to our Associate Medical Director. This role will be onsite and in-person at the Creekside Clinic in The Villages, FL.

Key Responsibilities

• Provide comprehensive primary care services to adult and geriatric patients, including assessments, diagnosis, treatment, and ongoing care management
• Manage chronic conditions with an emphasis on preventive care and wellness
• Perform physical exams, order and interpret diagnostic tests, and develop individualized treatment plans
• Collaborate closely with physicians, care coordinators, behavioral health specialists, and clinical support staff
• Focus on quality-driven outcomes, including HEDIS measures, CMS Star initiatives, and population health goals
• Educate patients and caregivers on disease management, medications, and healthy lifestyle choices
• Maintain accurate and timely documentation in the electronic medical record (EMR)
• Participate in care team meetings and quality improvement initiatives

Use your skills to make an impact
 

Required:

• Master's degree in nursing with board certification, or graduation from an accredited PA program with board certification.
• Active, unrestricted NP or PA license in the state of Florida, or the ability to obtain it
• Active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.

Preferred:

• At least one year of experience in value-based care, primary care, or Medicare Advantage settings.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

 

$123,800 - $170,300 per year

 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
