Pay: $54–$56 per hour
Schedule: PRN (Per Diem) – Weekends

Diablo Valley Post Acute is seeking a dedicated, compassionate, and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team in a PRN weekend position.

What You'll Do
As a PTA, you will work under the direction of a Physical Therapist to help residents improve their strength, mobility, balance, and functional independence.
Responsibilities include:
Provide skilled physical therapy interventions under the direction of a licensed Physical Therapist.
Assist residents in improving mobility, strength, balance, coordination, and functional abilities.
Implement individualized treatment plans and therapeutic activities.
Monitor resident response to treatment and communicate progress or concerns to the Physical Therapist.
Educate residents and caregivers on safe mobility, exercises, and therapeutic techniques.
Collaborate with nursing and other members of the interdisciplinary care team.
Maintain accurate and timely clinical documentation.
Provide compassionate, resident-centered care focused on achieving meaningful functional outcomes.
Qualifications
Current California Physical Therapist Assistant (PTA) license
Graduate of an accredited Physical Therapist Assistant program
Skilled nursing, post-acute, or rehabilitation experience preferred
Strong communication, organizational, and interpersonal skills
Ability to work independently while collaborating effectively with an interdisciplinary team
Commitment to compassionate, resident-centered care
