The Patient Navigator serves as a liaison for Jefferson patients. Specifically, the Patient Navigator will work with physicians and staff to ascertain the appropriate services the patient needs; schedule the patient appointments with multiple departments; and follow up with the patient to assure compliance. Summary
The Patient Navigator serves as a liaison for Jefferson patients. Specifically, the Patient Navigator will work with physicians and staff to ascertain the appropriate services the patient needs; schedule the patient appointments with multiple departments; and follow up with the patient to assure compliance. The Patient Navigator supports delivery of the Jefferson Experience by placing emphasis on the prompt and courteous treatment of all patients and visitors.

Job Duties

• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
• Schedules patients for all visits undergoing future care
• Administers screenings and intake assessments as needed
• Monitor treatment plan adherence
• Triage patient phone calls
• Coordinate inpatient care as needed
• Develop, maintain, update community resource list
• Greets and interacts with patients/visitors in a professional manner, on phone or in person.
• Adheres to HIPAA privacy and security requirements. Ensures compliance with all applicable federal, state, and local regulatory standards.
• Other duties as requested or assigned.

Minimum Qualifications

• 3 years of experience in a medical office required
• Must be able to communicate courteously with patients and/or visitors.
• Willingness to provide and support a positive Jefferson Experience for all patients and/or visitors.
• Leadership skills to include proactivity and problem solving.
• Ability to establish and develop positive and collaborative relationships.

Preferred Qualifications

• Bachelor’s Degree in psychology, health administration or other related fields from an accredited college/university
• Epic experience preferred

Additional Qualifications

Physical Demands
Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.

Salary Range

$20.58 to $29.34 Hourly

The actual hiring rate will be determined based on candidate experience, skills and qualifications. This position is not eligible for an annual incentive.
