We are seeking a Patient Care Specialist II to join our Home Health team. In this remote role, you will coordinate front office operations, scheduling, patient communications, and documentation activities that support efficient care center operations. You will utilize your knowledge of clinical support workflows to address routine patient access inquiries, maintain accurate patient records, manage appointment scheduling, and facilitate effective communication between patients, providers, and care teams.

The Patient Care Specialist II performs standard operational processes under moderate supervision, applying established knowledge and skills to tasks that may occasionally require interpretation or deviation from standard procedures. This role follows defined processes and guidelines, with decisions directly impacting team productivity and patient experience. Success in this position requires the ability to communicate information clearly, provide explanations when needed, and consistently deliver results within established expectations and oversight.

Key Responsibilities

• Coordinate patient scheduling, registration, visit preparation, and follow-up activities across assigned care center workflows.
• Verify patient demographics, insurance information, referral details, and required documentation prior to or during visits.
• Respond to patient calls, messages, and front desk inquiries, providing accurate information within established guidelines.
• Identify scheduling gaps, missing documentation, or incomplete patient information and take appropriate steps to resolve routine issues.
• Route clinical messages, refill requests, forms, and care-related inquiries to the appropriate care team members.
• Maintain accurate records in electronic medical record, scheduling, and patient communication systems.
• Support daily front office workflow by prioritizing routine tasks, coordinating with clinical staff, and escalating issues when needed.

Requirements

• High school diploma or GED required.

• 2+ years of progressive experience in a healthcare or patient access environment.

• Strong working knowledge of EHR systems, insurance verification, and medical terminology.

• Strong attention to detail, interpersonal communication, and time-management skills.

• Demonstrated professionalism and commitment to patient confidentiality.

• Demonstrates empathy and professionalism when interacting with patients and families.

• Adheres to HIPAA and privacy standards in all communications and record handling.

• Consistently meets expectations, follows direction, and maintains punctuality.

• Adjusts to changing workflows and patient volume with composure and teamwork.

• Demonstrated ability to manage multiple priorities while maintaining accuracy.

• Proven commitment to maintaining confidentiality, accuracy, and professional ethics.

• Ability to work independently in a fast-paced, cross-functional environment.

Compensation Range:

$17.10

to

$25.65

 

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
