What You'll Do
- Provide preventive, acute, and chronic care
- Partner with patients to create personalized care plans
- Collaborate with an interdisciplinary care team focused on whole-person health
- Use evidence-based medicine and population health strategies to improve outcomes
- Empower patients through education, coaching, and shared decision-making
What You'll Bring
- MSN from an accredited program
- Board Certified Nurse Practitioner
- Current RN/APRN license and DEA registration
- Primary care experience preferred
- Passion for relationship-based, patient-centered care

$178,968.00
The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
