About Pharmacy Medication History Surgical Optimization Team

The Surgical Health Optimization Center was developed to help patients properly prepare for their upcoming surgery. Our goal is to improve each patient's health in order to decrease their risk of complications during and after surgery. A preoperative physical exam helps your surgical team determine if you are healthy enough to undergo anesthesia and surgery. This is sometimes referred to as being optimized for surgery. Knowing a patient's medications, doses, and their schedule of administration is essential to appropriately instruct patients and surgeons on how to best manage them in consideration of underlying medical conditions and the proposed surgery.

Works alongside other Medication History Pharmacy Technicians in the Surgical Optimization Office located within Blodgett Hospital. Supports the Blodgett and Regional Offices by completing medication histories via telephone. Occasional in-office patient interviews may be necessary. Documents medication history in the medical record accurately and completely according to standard procedures, participates in data collection for service line justification, and remains open to change and innovation. Exercises excllent communication skills and professionalism in the workplace.

Scope of Work

Provides technical assistance in the documentation and clarification of medication history lists under the direction of the pharmacist. Processes, enters, and interprets physician/nurse medication histories, under the direction of the pharmacist, and completes full history documentation in computerized system. Clarifies information through communication with providers, patients, and pharmacies. Performs special projects for the department. Works with multiple medication automation systems.

• Provides professional customer service as a liaison between the pharmacy department and patients/caregivers. Interacts with prescriber as needed.
• Processes physician/nurse/pharmacist obtained patient histories and enters into the computer system and assists in maintaining patient profiles.
• Reviews profile for allergies, duplication of drugs, and last doses and brings these to the attention of the pharmacist.
• Checks and verifies accuracy of patient profiles.
• Maintains clean and organized work area.
• Maintains all requirements for CPhT certifications. Completes all Corewell Health assigned competencies by assigned due dates.
• Perform quality assurance data collection, collation, and presentation.

Qualifications

• Required High School Diploma or equivalent
• Preferred Associate's Degree health care or related field
• 1 year of relevant experience Pharmacy Required
• 2 years of relevant experience Retail Pharmacy Preferred
• LIC-Pharmacist Intern - STATE_MI State of Michigan Upon Hire required Or
• LIC-Pharmacy Technician - STATE_MI State of Michigan Upon Hire required And
• CRT-Pharmacy Technician, Certified (CPHT) - PTCB Pharmacy Technician Certification Board 90 Days required Or
• CRT-Exam for the Certification of Pharmacy Technicians (ExCPT) - NHA National Healthcareer Association 90 Days required

• Ideal candidate will possess Full Pharmacy Technician License and National Certification, such as PTCB or ExCPT

• This is a Part Time Position offering Full Benefits

• Schedule will be weekdays, Mon/Tue and every other Fri or Tue/Thu and every other Fri.
