Calvary Hospice provides end-of-life care to patients with life-limiting illnesses whose care has transitioned from active curative to palliative and quality of life focused. The majority of care is provided in the patient’s home and as needed, short term inpatient care is provided at several facilities throughout the greater New York area.

Requirements

Maintains and coordinates the flow of information received in the referral process.  Communicates with patient, family and appropriate staff to ensure accurate and timely insurance verification process.  Is responsible for maintaining data entered in to the computer system.

• Serve as the primary point of contact for incoming hospice referrals from hospitals, physicians, facilities, and community sources
• Collect and document complete referral and demographic information; initiate contact with patients/families to explain the hospice admission process
• Coordinate with clinical staff to facilitate eligibility review, including physician certification and verbal hospice orders; escalate clinical questions to RN as appropriate
• Verify insurance coverage and benefits (Medicare, Medicaid, managed care, commercial plans), determine authorization requirements, and obtain pre-authorizations when needed
• Collaborate closely with Finance to ensure timely and accurate insurance verification and documentation
• Clearly explain hospice benefits, coverage, and required documentation to patients and families; communicate any coverage limitations to leadership
• Maintain accurate, real-time documentation of referral status, pending admissions, and authorization updates in the EMR/clinical system
• Monitor and follow up on pending referrals , ensuring timely progression to admission
• Coordinate and schedule hospice admissions 

Salary: $58,124.92 /annually

Qualifications:

• Minimum of four years experience in a coordinator or related capacity is required.
• Experience in a Hospice, Home Health Agency or Hospital is preferred.
• Competency with computers is required.  A high level of competence with computer software such as Word and Excel as well as medical terminology.
• Experience in intake and customer service activities is preferred.
• Excellent telephone etiquette required.
• Bi-lingual language skills preferred.
• Ability to maintain confidentiality and highly developed organizational skills.
• Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.

Education:

• High School Diploma or equivalent is required.
