Compensation is commensurate with experience starting at $42/hr.
What You’ll Do:
- Provides direct patient care.
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Collects assessment data and performs observation to report to and collaborate with RN(s) and physician(s).
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse and/or Director of Clinical Services.
- Uses equipment and supplies effectively and efficiently.
- Participates in personal and professional growth and development.

- Licensed Practical Nurse (LPN) licensure in Oregon
- 1+ Year of Nursing Experience
- Ability to shine solo or rock it out as part of a dynamic team

- Hold a current BLS certification from AHA
