Role Scope: The Functional Wellness Supervisor leads and directly oversees the rehab team which includes Physical Therapists (PT), Occupational Therapists (OT), Aides, and Home Care Coordinators at the PACE center. This role ensures that therapy interventions are high-quality, participant-centered care that supports independence, safety, and quality of life. The Functional Wellness Supervisor facilitates functional wellness programming, performance improvement, and cross-departmental collaboration. This role is responsible for the day-to-day team supervision, training and competency oversight, and regulatory compliance for the rehab team working in close partnership with Center leadership and the interdisciplinary team.
Core Responsibilities & Expectations for the Role
- Lead rehabilitation and functional wellness services at the Center to support independence, safety, and quality of life.
- Provide oversight for Occupational Therapy, Physical Therapy, and integrated functional wellness programming.
- Align clinical operations with PACE regulations, CMS requirements, state licensure standards, and organizational goals.
- Partner with the interdisciplinary team and center leadership to ensure clinically appropriate, evidence-based, participant-centered interventions and care plans.
- Manage staffing, scheduling, and workflows; coach performance and ensure reliable service coverage.
- Monitor key performance indicators (utilization, functional outcomes, and participant satisfaction) and drive quality improvement through audits and corrective actions.
- Build a data-driven practice by standardizing functional outcome measures, strengthening documentation of measurable progress, and creating reporting/dashboards that translate outcomes into operational and clinical insight.
- Support efforts to reduce preventable hospitalizations through proactive functional interventions and coordinated care.
- Lead fall prevention and recovery initiatives: screening and risk assessment, individualized and group interventions (balance/strength training, environmental modification, caregiver education), incident analysis/root cause review, and coordination with nursing and medical providers.
- Champion innovation and continuous learning by integrating current geriatric rehabilitation research, piloting new programs/tools, evaluating impact, and scaling best practices across PACE centers.
- Emphasize functional wellness to reduce acute utilization, recovery after hospitalization/illness and long-term aging-in-place.
Required Qualifications:
- Unrestricted Occupational or Physical Therapy license in the state of practice (or eligible for licensure) with a bachelor’s degree from accredited program.
- Two to four years of experience with at least one of those leading people including performance coaching, scheduling and workflow management.
- Demonstrated experience in geriatric rehabilitation, functional assessments, and evidence-based interventions.
- Strong knowledge of PACE regulations, CMS requirements, and state licensure standards.
- Experience developing or overseeing functional wellness programs, fall-prevention initiatives, and aging-in-place strategies.
- Ability to interpret and act on clinical and operational data, including functional outcome measures and KPIs driving quality improvement, innovation, and continuous learning.
- Skilled in interdisciplinary collaboration and participant-centered care planning.

Compensation:
The anticipated base salary range for this position is $117,000 - $144,000. This position is also eligible for a target annual bonus and may be eligible for additional incentive compensation, as applicable.
The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
