⭐ About the Role

Location: Remote – Canada based

We're hiring a full-time remote Advocacy Care Navigator to serve as a trusted guide for existing Sailor Health patients who are ready to take the next step in their care journey through our advocacy services. In this role, you'll connect with patients who have already begun working with a therapist and support them in accessing the additional resources, coordination, and advocacy they need to thrive.

This role blends patient support, care coordination, and emotional guidance. You'll be the steady, familiar voice that helps an established patient feel truly supported beyond their therapy sessions - walking them through advocacy intake with warmth, clarity, and close attention to detail.

It's a great fit for someone who finds meaning in continuity of care and who thrives at the intersection of empathy and execution.

💻 Your Key Responsibilities

• Conduct intro calls with existing patients who are interested in Sailor Health's advocacy services, following a similar structure to care navigation intake conversations

• Guide patients through the advocacy onboarding process with warmth, clarity, and attention to detail

• Coordinate closely with the advocacy team to ensure smooth transitions and follow-through

• Manage patient communications across multiple stages of the process, including follow-ups, missed calls, and reassignments

• Work within tools like Google Workspace, EHR platforms (e.g., Healthie), and CRMs like HubSpot to keep records accurate and up to date

• Tech-comfortable. You pick up new tools quickly and are comfortable navigating platforms like Google Workspace, EHR systems, and CRMs.

• Mission-aligned. You care deeply about improving care for older adults and are excited to play a meaningful role in their ongoing support.

• Experience welcome, not required. A background in healthcare, customer service, or administrative support is a plus — but above all, we're looking for someone organized, compassionate, and eager to grow.
