Join our team and make a positive impact on the lives of others! We are looking for a Care Care Manager (RN) for our Georgia team.
This is a full time field-based position that requires frequent travel to patient's homes in Jonesboro, Medonogh, Lawrenceville and Moreland counties.
Department & Position Overview:
FreedomCare’s Georgia operations support Structured Family Caregiving (SFC), a Medicaid home and community-based service option that helps eligible individuals receive daily care from a qualified live-in family caregiver, with coaching and oversight to support safe care at home.
The Care Manager, RN collaborates closely with patients, caregivers, and interdisciplinary care teams to assess needs, develop individualized plans of care, and facilitate the seamless initiation and delivery of services. Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance. The Care Manager, RN proactively communicates clinical concerns, changes in condition, and potential barriers to care with the appropriate team members, Case Management Agency, to ensure timely intervention, effective care coordination, and optimal patient outcome in addition to providing education to caregivers and patients to ensure optimal patient outcomes.
Every Day You Will:
- Conduct initial and semi-annual in person visits, in-person RN supervisory visits and coordinate timely service activation, ensuring all required documentation, approvals, and standard procedures are completed.
- Complete self-scheduling for any in person visits according to company policy.
- Complete a minimum of 16-20 in person visits weekly. Fluctuation may occur dependent on established needs and territory.
- Complete any in-person related visit documentation or care coordination the same business day of in-person visit.
- Follow safety protocols in the field including PPE use when indicated, PPE. Follow equipment and privacy rules per company policy. Follow HIPAA laws to ensure patient privacy is upheld.
- Ability to drive self independently to and from place of residence to patient’s residence to complete required in-home visits. Drive time can be up to 2 hours, but occasionally there may be a need outside your assigned territory dependent on need.
- Develop, implement, and manage individualized coaching care plans, making and documenting revisions as the patient’s needs or condition change.
- Educate patients, caregivers, and internal teams regarding care needs, safety considerations, service requirements, caregiver responsibilities, and next steps.
- Coordinate patient care through timely communication and documentation with clinical disciplines, internal partners, case management agencies and appropriate community resources.
- Provide ongoing education and coaching to support effective use of established best practices, promote caregiver competency, and connect patients and families with appropriate resources.
- Provide SFC-specific clinical consultation and guidance to Health Coaches, Care Specialists, and caregivers when health concerns, caregiver training needs, documentation issues, safety concerns, care gaps, or changes in the patient’s condition are identified.
- Review caregiver daily notes and related care documentation for each assigned active SFC case a minimum of every 14 calendar days. Document completion of each review through a signature, electronic authentication, or other auditable method and respond appropriately to identified health, safety, environmental, documentation, or service concerns.
- Identify and timely escalate changes in condition, health or safety concerns, unmet care needs, service gaps, incidents, and other urgent concerns in accordance with established clinical protocols. Coordinate applicable concerns with Case Management including any identified concerns noted through assessments, visits, clinical observations, care-plan updates, incidents, daily notes, communications. The RN must complete follow-up actions accurately, thoroughly, and within required timeframes.
- Complete requirements using only approved methods and systems of documentation, phones calls, and systems for customer outreach (Salesforce, TalkDesk, Email, Teams, CareConnect, MedFlyt).
- Identify clinical process gaps, documentation risks, recurring concerns, and opportunities to strengthen SFC oversight, consultation, escalation, and compliance-monitoring workflows. Support policy implementation, compliance reviews, audit preparation, record validation, and corrective follow-up related to assigned SFC clinical-oversight responsibilities.
- Maintain compliance with applicable federal and Georgia requirements, Medicaid and EDWP program requirements, CCSP and SOURCE requirements, SFC service requirements, professional nursing standards, and company policies and procedures.
- Attend Team meetings virtually or in-person as requested.
- Responsible for escalating any unsafe or concerning customer or staff issues immediately per company policy
- Perform other related duties as assigned by the supervisor.
Credentials/Certifications:
- Diploma from an accredited nursing program; valid and active Registered Nurse (RN) license in the State of Georgia. (Georgia compact RN licenses are allowed)
- A minimum of two years of recent nursing experience supporting adults or geriatrics and/or adults with disabilities preferred.
- Experience supporting Medicaid waiver programs, home and community-based services, clinical documentation review, care coordination, compliance monitoring, or clinical escalation workflows preferred.
- Valid driver’s license and car insurance required, with means of reliable transportation.
- Valid CPR/first aid certification must be active or able to be obtained prior to starting position

- Strong typing and computer proficiency, with the ability to efficiently navigate contact center technology, customer relationship management systems, telephone platforms, and other necessary software applications while accurately documenting interactions.
- Effective problem-solving and critical thinking, with the ability to evaluate information, identify appropriate solutions, exercise sound judgment, and escalate concerns when necessary.
- Dependable and punctual, with a demonstrated commitment to consistent attendance, schedule adherence, and performance expectations.
- Ability to adapt to changing priorities, processes, systems, and business needs in a fast-paced environment.
- Receptive to coaching and feedback, with the ability to apply guidance and demonstrate continuous improvement.
- Ability to collaborate effectively as part of a team while working independently within established guidelines.
- Strong clinical judgment and critical thinking skills.
- Proficiency with electronic documentation systems, customer relationship management platforms, Microsoft Office applications, and other systems used to complete and document clinical-oversight activities.

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $80,000 and $90,000 per year at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
