Home Health RN Responsibilities

Provide skilled home health nursing care, including assessments, wound care, medication management, and treatments
Develop and implement individualized home health care plans
Monitor patient progress and adjust care plans accordingly
Educate patients and families on disease management, rehab, and senior care
Coordinate care with physicians, therapists, and interdisciplinary teams
Complete accurate and timely documentation in compliance with home health regulations

Qualifications – RN Home Health Jobs

Active California Registered Nurse (RN) license (required)
Minimum 1 year of RN experience (home health experience preferred)
Experience in home health, rehab, geriatric care, or community care is a plus
Strong clinical, communication, and organizational skills
Valid driver’s license and reliable transportation
Ability to travel within Los Angeles County (Whittier, Lakewood, Santa Fe Springs & surrounding areas)

Preferred Qualifications

Prior RN Home Health experience
Public Health Nurse (PHN) certification
Experience with mobile nursing or home-based care models
