π©Ί Primary Job Responsibilities
- Delivers proficient nursing care in accordance with agency policies and the Pennsylvania Nurse Practice Act.
- Performs comprehensive initial and ongoing assessments, including OASIS, to evaluate patient needs and familial support.
- Determines the necessary level of care required to achieve the desired patient outcomes.
- Provides education and counseling to patients and their families concerning nursing care and related home-care requirements.
- Initiates and revises individualized care plans as needed.
- Informs physicians and care team members of any changes in the patient's condition.
- Oversees LPNs, HHAs, and HMKs as necessary.
- Refers patients to suitable agencies for additional or supplementary services.
- Submits documentation precisely and punctually in compliance with agency policies.
- Maintains up-to-date nursing expertise through ongoing education, training, and professional development.
- Full-time RNs are expected to complete 30β32 visits per week.
Requirements
- Current RN license in good standing to practice in the Commonwealth of Pennsylvania
- Maintains clinical and professional competence
- CPR certified
- Driver's License and Car Insurance in good standing
- Reliable vehicle
- Possesses the ability to follow oral and written instructions
- Possesses the ability to communicate effectively with all members of the care team
- Possesses good organizational skills.
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
