The role of a Mental Health Specialist within our Programs in Assertive Community Treatment (PACT) focuses on integrating clients into the community through personalized support and engagement in various community settings. Key Responsibilities:

• Under supervision:
• Provides supportive assistance with activities of daily living (ADLs) through modeling, direct assistance, and coaching.
• Assists clients in obtaining basic needs.
• Assess client emotional well-being, clinical symptomology, behavioral changes and medication effectiveness.
• Provides psychosocial education for consumers related to mental illness, symptom management and medication compliance.
• Provides crisis intervention as necessary.
• Provides rotating 24 hour on call coverage as scheduled.
• Maintain relationships with state and local hospitals.
• Participate in daily treatment team meetings and weekly treatment team planning meetings.

Requirements

• High School Diploma or equivalent
• Bachelor's Degree preferred
• One year of experience in the provision of social services for individuals with severe persistent mental illness
• Some lived experience
