Position Title: Bayside Team Lead
Position Type: Full Time; 35 Hours/Weekly

Salary: $42,000 - $47,000
Responsibilities:
- Provide case management services to an assigned caseload, including follow-up and care coordination.
- Facilitate psychoeducational, skill-building, and therapeutic groups for consumers and families.
- Facilitate individual and group supportive counseling sessions for program consumers.
- Develop, implement, and review individualized treatment plans within required timeframes. Conduct outreach to consumers who miss scheduled program days through phone or face-to-face contact.
- Collaborate with external providers to support continuity of care and comprehensive treatment planning.
- Complete daily and weekly clinical documentation in compliance with agency standards and timelines.
- Participate in multidisciplinary treatment team meetings to support coordinated consumer care.
- Provide crisis intervention services and coordinate referrals to emergency psychiatric services as needed.
- Support and train newly hired case managers on program procedures and best practices.
- Participate in process improvement initiatives, committees, and assigned organizational projects.
- Provide transportation of youth to and from program as needed utilizing agency vehicles.
- Assist and support program operations in absence of Program Coordinator.
Requirements
- Bachelor or Master's Degree in Social Work or related field
- LSW Preferred but not required (supervision available towards clinical licensure)
- Minimum of two years experience working with children/adolescents in a mental health setting
- NJ Driver's License with less than 6 points
