East Bay Post Acute is seeking a compassionate and motivated Physical Therapist (PT) to join our dedicated in-house rehabilitation team.
As a Physical Therapist, you’ll help residents regain strength, mobility, balance, and independence while providing high-quality, individualized care. Key Responsibilities
As a Physical Therapist, you will:
Perform comprehensive physical therapy evaluations and develop individualized treatment plans
Provide skilled, evidence-based interventions including therapeutic exercise, gait training, balance activities, functional mobility training, and manual therapy
Monitor resident progress and adjust treatment plans to maximize functional outcomes
Collaborate with physicians, nursing staff, therapy professionals, and the interdisciplinary care team
Educate residents and families on therapeutic exercises, mobility techniques, safety, and discharge recommendations
Participate in care planning and discharge planning as appropriate
Maintain accurate, timely, and compliant clinical documentation
Qualifications
Current and active California Physical Therapist (PT) license
Strong clinical assessment and treatment-planning skills
Excellent communication and interpersonal skills
Ability to work effectively as part of an interdisciplinary team
Commitment to compassionate, resident-centered care
Strong organizational and documentation skills
