General Summary:

The LTC Wellness Manager is a strategic clinical and program leader responsible for delivering, evolving, and scaling proactive long-term care wellness programs that help policyholders live healthier, longer, and more independent lives. Operating with a high degree of autonomy, the role translates member needs, business priorities, operational insights, and performance data into scalable solutions. The role influences priorities and decisions without direct authority and serves as a subject matter expert in aging, caregiver support, care navigation, functional risk, and aging in place.

This Wellness Program Manager is responsible for delivering and evolving proactive long-term care wellness programs designed to help policyholders live healthier, longer, and more independent lives. The role combines clinical assessment, member engagement, care navigation, and program operations to support aging in place, reduce risk, and improve outcomes across both pre-claim and emerging on-claim wellness offerings.

Principal Duties and Responsibilities

  • Own the execution, ongoing evolution, and continuous improvement of complex, cross functional LTC wellness programs, ensuring alignment with LTC strategy, member needs, risk objectives, and business priorities.
  • Operates independently within broad strategic objectives and makes recommendations that may affect program design, vendor performance, member experience, operating processes, and investment priorities.
  • Balances clinical quality, member needs, operational feasibility, risk, compliance, cost, and expected business value when recommending a course of action.
  • Develop and maintain program roadmaps, implementation plans, operating models, workflows, decision frameworks, and measures of success for new and existing wellness capabilities.
  • Ensure that wellness programs are implemented and operated with clear objectives, accountable owners, scalable processes, and measurable outcomes and that Members and caregivers are effectively connected to relevant services and receive a consistent, high-quality experience.
  • Use clinical, functional, cognitive, behavioral, environmental, and social insights to shape member journeys, assessment approaches, service models, eligibility concepts, and intervention strategies and are informed by member feedback, operational evidence, analytics, and financial considerations.
  • Conduct or oversee telephonic engagement and assessments for higher-risk or complex members and translate direct member experience into scalable program and process improvements.
  • Establish and monitor key performance indicators such as engagement, referral conversion, service utilization, completion, member experience, operational performance, risk outcomes, and financial value.
  • Partner with Analytics and Finance to interpret program results, identify trends, test assumptions, evaluate effectiveness, and translate findings into clear recommendations for leadership.
  • Serve as the business owner or primary operational lead for designated wellness vendors; define expectations, contribute to vendor selection monitor performance, lead governance discussions, resolve escalated issues, and drive corrective actions and service improvements.
  • Partner with Product, Technology, Digital Experience, and operations teams to translate business and member needs into requirements, prioritized enhancements, acceptance criteria, and implementation decisions.
  • Leads work across multiple functions and external partners where authority is shared and successful delivery depends on influence, judgment, and sustained stakeholder alignment.
  • Lead operational readiness for new programs and material changes, including process design, communications, training, scripting, controls, documentation, and post-launch feedback loops.
  • Provide concise, data-informed updates and recommendations to senior leaders and stakeholders; facilitate decisions involving competing priorities, ambiguity, operational constraints, and member impact.
  • Ensure LTC leadership has clear visibility into program performance, business value, delivery risks, and recommended decisions.
  • Develop standards, playbooks, assessment guidance, and quality expectations for wellness delivery; provide subject matter guidance, coaching, and informal mentorship to colleagues and future care navigation resources.
  • Ensure programs and member interactions are executed in accordance with applicable legal, privacy, compliance, clinical, information security, and vendor-management requirements.
  • May perform other duties as assigned.

Job Specifications

  • Bachelor's degree required in Occupational Therapy, Nursing, Social Work, Gerontology, Public Health, Rehabilitation, Case Management, Health Administration, or a related clinical or health discipline; equivalent relevant experience may be considered.
  • Master's degree in a related clinical, public health, business, or health administration discipline preferred.
  • Active licensure as an Occupational Therapist (OTR/L) strongly preferred. Physical Therapist (PT) licensure also highly valued. Candidates must demonstrate expertise in functional assessment, aging in place, home safety, caregiver support, mobility, and independence.
  • 7+ years of progressive experience in clinical services, care management, wellness, aging services, rehabilitation, population health, insurance, or long-term care.
  • 3+ years of experience leading complex programs, implementations, pilots, products, operational improvements, or cross-functional initiatives from strategy through execution.
  • Demonstrated experience influencing stakeholders across business and technical functions without direct authority and communicating recommendations to senior leaders.
  • Experience using performance data, member or customer insights, and financial considerations to evaluate effectiveness and make business recommendations.
  • Experience managing external vendors, service partners, contracts, implementations, or performance governance preferred.
  • Demonstrated ability to create structure in ambiguous environments, manage competing priorities, identify risks and dependencies, and drive work to measurable outcomes.
  • Advanced knowledge of aging, functional decline, caregiver needs, home safety, community resources, chronic conditions, cognitive impairment, and care navigation.
  • Strong business acumen, critical thinking, executive communication, facilitation, negotiation, and problem-solving skills.
  • Ability to translate clinical and member needs into scalable operating processes, product requirements, service standards, and program enhancements.
  • Proficiency with Microsoft Office applications; experience with CRM, workflow, case management, analytics, or digital engagement platforms preferred.

The base salary range for applicants for this position is listed below. Unless actual salary is indicated above in the job description, actual pay will be based on skill, geographical location and experience.

$75,500.00-$142,700.00
