At St Vincent's Health Australia, we're redefining healthcare through our Care at Home portfolio, combining clinical excellence, digital innovation and multidisciplinary collaboration to:

  • Deliver hospital-level care beyond the hospital.

  • Improve recovery outcomes while reducing unnecessary hospital stays.

  • Increase patient independence, choice and quality of life.

  • Build a more sustainable healthcare system for future generations.

This is more than a new model of care. It's one of the most significant transformations in Australian healthcare.

Our Rehab at Home program is an established, evidence-based model that combines expert in-home physiotherapy, virtual care and multidisciplinary coordination to deliver exceptional rehabilitation outcomes beyond the traditional hospital setting.

Supporting patients recovering from elective orthopaedic surgery at St Vincent's Private Hospitals, the program enables safe early discharge, promotes faster recovery and reduces unnecessary hospital stays, allowing patients to rehabilitate in the comfort, familiarity and independence of their own homes.

As part of St Vincent's Care at Home vision, Rehab at Home is redefining rehabilitation by combining clinical excellence, innovative technology and compassionate, person-centred care to deliver better outcomes for patients and the broader health system.

  • Collaborate with medical, nursing and allied health professionals to optimise patient outcomes.

  • Support early discharge and hospital avoidance initiatives.

  • Maintain accurate clinical documentation and contribute to quality improvement activities.

  • Champion patient-centred, evidence-based rehabilitation practice.
