Shadelands Post Acute is seeking a compassionate and skilled Occupational Therapist (OT) to join our dedicated in-house rehabilitation team.

Key Responsibilities
As an Occupational Therapist, you will:
Evaluate residents and develop individualized occupational therapy treatment plans
Provide skilled OT interventions focused on ADLs, functional mobility, safety, and independence
Monitor resident progress and modify treatment plans as appropriate
Supervise and provide clinical guidance to COTAs
Collaborate with nursing, physicians, therapy professionals, and other interdisciplinary team members
Participate in care planning and discharge planning
Educate residents, families, and caregivers on treatment strategies and techniques
Maintain accurate, timely, and compliant clinical documentation
Qualifications
Current and active California Occupational Therapist (OT) license
Strong clinical assessment and treatment-planning skills
Excellent communication and interpersonal skills
Ability to work effectively within an interdisciplinary team
Commitment to compassionate, resident-centered care
Weekend availability preferred
