We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.

About the role

As a Veterans Case Manager for the Housing Pathways Program Prisons, you will play a vital role in supporting veterans who are experiencing or at risk of homelessness, helping them rebuild stability, independence, and hope for the future. Through compassionate, person-centred case management, you will connect veterans with housing, support services, and community resources, addressing the barriers that impact their wellbeing and long-term outcomes. This is a rewarding opportunity to make a meaningful difference in the lives of those who have served, empowering individuals to secure safe housing and create positive, lasting change.


Reporting to the Team Leader, this is a fixed-term full-time position, contracted for 12 months. As this is an outreach role you will be based in the Sunshine Hub, based on-site in Sunshine, VIC. This role will require ad hoc attendance at various prisons, offsite meetings and assisting clients out in the community.

Conditions
Compensation is in accordance with the SCHADS Award Level 5.

How you will make an impact

  • Support veterans at risk of homelessness to access suitable housing options and navigate the housing system
  • Deliver person-centred case management, including goal planning, advocacy, and coordination of support services to aid in a successful reintegration
  • Connect clients with community resources, employment, training, health, and wellbeing services to promote long-term stability
  • Build and maintain strong partnerships with correctional services, housing providers, and key community stakeholders
  • Manage client records, risk assessments, and service delivery in line with organisational standards, legislation, and homelessness service frameworks


What you will bring

  • Relevant tertiary qualifications in Social Work, Community Services, or a related field (degree preferred), or equivalent experience
  • Understanding of the veteran community, including experience within the Australian Defence Force or knowledge of the unique challenges faced by veterans
  • Knowledge of the homelessness sector, including relevant legislation, frameworks, and best-practice approaches to supporting people experiencing or at risk of homelessness
  • Strong case management skills, with the ability to assess needs, build effective support plans, and deliver person-centred outcomes
  • Experience working collaboratively with stakeholders, service providers, and community networks to achieve positive client outcomes
  • Experience within the justice or correctional services sector will be highly regarded
  • A current and valid Victorian Driver's Licence
  • A current Employee Working with Children Check
