Liquid Personnel is recruiting Social Worker for its client's Hospital Discharge Team in Cornwall.

This is an excellent opportunity for a dedicated practitioner to support adults with complex care and support needs through safe, timely, and effective discharge planning from hospital settings.

The successful candidate will work within a busy and supportive multidisciplinary team alongside health professionals, families, carers, and community services to promote independence, wellbeing, and positive outcomes. You will play a key role in ensuring individuals receive appropriate support following discharge, undertaking statutory assessments and coordinating services that enable people to return to the most appropriate setting whilst reducing delays and preventing unnecessary hospital admissions.

This role requires a confident Social Worker with strong assessment skills, sound knowledge of Adult Social Care legislation, and the ability to manage complex decision-making in a fast-paced hospital environment.

What will your responsibilities be?

• Supporting safe, timely, and person-centred hospital discharge planning for adults with complex care and support needs.
• Undertaking Care Act 2014 assessments, reviews, and reassessments to identify eligible social care needs and support discharge arrangements.
• Completing Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
• Supporting and reviewing individuals within Discharge to Assess (D2A) placements to ensure ongoing support arrangements remain appropriate.
• Arranging and coordinating care packages, community support services, equipment, and placement options to facilitate successful discharge outcomes.
• Working collaboratively with hospital staff, health professionals, carers, families, and partner agencies to achieve positive outcomes for individuals.
• Managing a varied caseload and prioritising referrals to ensure timely progression through hospital discharge pathways.
• Undertaking safeguarding interventions and responding to safeguarding concerns in accordance with statutory guidance and local procedures.
• Assessing and managing risk effectively, ensuring discharge plans balance independence, safety, and individual choice.
• Supporting adults and carers through periods of transition, providing advice, guidance, and advocacy where appropriate.
• Contributing to multidisciplinary meetings, discharge planning meetings, safeguarding discussions, and case reviews.
• Maintaining accurate, timely, and legally compliant records, assessments, reports, and support plans.
• Supporting cases involving complex legal frameworks, including Court of Protection matters where required.

Qualifications & Experience:

To be successful in this role you must have, 

• Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
• Registration: Must be registered with Social Work England.
• Experience: Significant post-qualified experience working with adults within Adult Social Care services.
• Hospital Experience: Previous experience within a hospital discharge team, acute setting, or Discharge to Assess pathway is highly desirable.
• Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
• Assessment Skills: Proven ability to undertake Care Act Assessments, Mental Capacity Assessments, risk assessments, and support planning.
• Court of Protection: Experience of Court of Protection work is desirable.
• Multi-Agency Working: Strong experience working collaboratively with NHS professionals, community services, care providers, families, and carers.
• Communication Skills: Excellent communication, negotiation, report-writing, and decision-making skills.
• Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
• Organisational Skills: Ability to work independently, manage competing priorities, and perform effectively within a busy hospital environment.
