Senior Care Co-Ordinator

Location: Bryson Court/Livingston
Contract: Permanent / Full Time 40hrs a week
Salary: £35,000 per annum, plus company benefits

We aim to work in partnership with commissioners and others to develop and deliver high-quality care in the community to vulnerable people. Our goal is to provide housing with care based on an enabling approach with an outcome focused support planning process to meet individual service user needs.

As a large provider of social care services, Mears works collaboratively with multi-disciplinary teams to address challenges in the social care marketplace.

We are Mears Supported Living Ltd, providing a range of care and support services to people out in the community and in their own homes.

About the Role:

As a Senior Care Co-Ordinator you will be an exceptional leader, with the ability to make a positive difference to people's lives. You will oversee the day-to-day operation of our Supported Living Services at Bryson Court, providing strong leadership to your team of Senior Key Workers and support workers while ensuring the delivery of high-quality, person-centred care. You will participate in multi-disciplinary assessments of service user needs and work closely with individuals, families, guardians and professionals to achieve the best possible outcomes.

The successful Senior Care Co-Ordinator will have a wealth of knowledge and experience supporting individuals with complex mental health needs, learning disabilities, autism and behaviours that challenge. You will take the time to understand individuals, and their specific support needs, working with their key workers to ensure continuity, consistency and positive outcomes.

You will be responsible for ensuring all care plans are reviewed on a six-monthly basis, observations and staff supervisions are undertaken quarterly, along with completing annual appraisals.

You will oversee the service users weekly planners to ensure meaningful, person-centred activities and support is delivered in line with agreed outcomes ensuring positive community engagement.

Role Responsibilities:

  • Lead, motivate and manage a team of Senior Key Workers and Support Workers, creating a positive and high-performing culture

  • Provide direct line management for Senior Key Workers and support development, ensuring they are effectively achieving the individuals’ personal outcomes

  • Complete and manage the staff rotas, ensuring services are safely staffed while maintaining continuity and consistency of care

  • Working closely with core support teams aligned to individual service users, ensuring staff develop a thorough and meaningful understanding of the individuals

  • Ensure individual weekly planners are completed, reviewed and reflect meaningful, outcome-focused activities tailored to each person's needs and aspirations

  • Support individuals with complex mental health needs, learning disabilities, autism and behaviours that challenge by promoting positive behaviour support and person-centred approaches

  • Identify specialist care needs based upon clinical and support requirements, monitoring and reviewing in line with agreed outcomes

  • Liaise with external organisations and professionals to achieve a comprehensive package for our service users

  • Reporting to the Supported Living Manager on a regular basis regarding service performance, staffing and outcomes for individuals.

  • Provide written reports for complaints, accidents, incidents and safeguarding concerns in line with company policies and procedures.

  • Undertake "on call" duties as required by the service to ensure a 24/7 cover is provided.

Key Criteria:

  • Previous experience within the care sector, ideally in a leadership or coordination role.

  • Extensive knowledge and experience supporting people with complex mental health needs, learning disabilities, autism and behaviours that challenge

  • Proven ability to lead, develop and motivate teams to deliver high-quality, person-centred care. Experience in use of behavioural strategies and de-escalation techniques

  • Experience managing rotas, staffing and service delivery

  • Strong understanding of outcome-focused support planning and person-centred practice.

  • Minimum SVQ Level 2 and willingness to complete SVQ 3

  • IT literate – Experience of using Word and Excel
