Summary:

Purchase District Health Department is seeking a knowledgeable and highly motivated individual to join the Rural Community Hubs for Chronic Care Innovation (RCH) program in the Purchase Area Development District.

Overview
This full time, term limited role is part of the Rural Health Transformation–RCH program, with Purchase District Health Department serving as the RCH Lead Organization for Purchase Area Development District and the Foundation for a Healthy Kentucky serving as statewide, neutral convening and implementation partner for the RCH program. For more context about the overall project, click here.

The Rural Community Hub (RCH) Operations Lead is responsible for operational management, partner engagement, financial oversight, and technical assistance within the Purchase Area Development District Hub. This role supports a statewide, multi-sector collaborative initiative to improve the health and wellbeing of rural Kentuckians.

The RCH Operations Lead will report to the Purchase District Health Department and work closely with the FHKY RCH Coordinator to help establish and scale Hub operations by developing foundational infrastructure, coordinating partners, supporting program implementation, and maintaining effective operational systems. As the initiative matures, the role will focus on optimizing workflows, supporting advancing ADD-level strategies that align local priorities with state goals.

Success in this position requires strong project management skills, experience coordinating multiple stakeholders, and the ability to translate strategy into effective operational implementation.

Why This Role Matters: This role is a unique opportunity to be part of a long-term, cross-sector effort to improve community health and wellbeing through coordinated local action. In a time when siloed systems struggle to meet complex community needs, Rural Community Hubs aim to align healthcare, public health, social services, education, housing, workforce, and community resources around shared goals and outcomes. The RCH Operations Lead plays a critical role in building the operational foundation necessary for these partnerships to function effectively and deliver measurable impact for Kentuckians.

About RHT

This investment will empower Kentucky to launch and implement its Rural Health Transformation Plan (RHTP) — a community-driven strategy to expand access and improve health outcomes for rural residents across the Commonwealth. Read Governor Beshear’s press release.

This project is 100% funded by the Centers for Medicare and Medicaid Services of the U.S. Department of Health and Human Services (HHS) for a total of $212.9 million in budget year 1 with 100% by CMS/HHS. The RHTF grant spans five budget periods aligned with federal fiscal years (FY2026–FY2030). Funding is disbursed annually by CMS based on achievement of performance metrics.
