As a Transitional Care Manager, you will help strengthen the connection between hospital-based care and the support individuals and families need as they transition to care at home.
This is an exciting opportunity to join a growing team supporting the downtown Jacksonville market. You will build trusted relationships within a large hospital campus, serve as a knowledgeable clinical resource, and help connect patients and referral partners with high-quality care and support.
The Opportunity
The Transitional Care Manager will be highly visible and active within the hospital environment. This role is ideal for an energetic Registered Nurse who enjoys connecting with people, building relationships, and becoming a trusted resource for case managers and other key partners.
You will spend the majority of your time on-site, developing strong relationships with hospital staff and referral partners. Success in this role requires comfort working in a fast-moving campus environment, maintaining a consistent bedside presence, and engaging with people throughout the day.
What You’ll Do

Establish and maintain strong, professional relationships with hospital case managers, clinical teams, and other referral partners.

Serve as a trusted clinical resource within the hospital and surrounding care environment.

Maintain a visible, consistent presence throughout the assigned downtown Jacksonville hospital campus.

Help connect patients and families with appropriate BAYADA services and resources.

Collaborate closely with Transitional Care Managers, Marketing Managers, and other members of the local team.

Identify opportunities to strengthen care transitions and support continuity of care.

Represent BAYADA with professionalism, compassion, and a strong commitment to reliability and excellence.
What You’ll Bring

An active, current Registered Nurse license in the state of Florida.

Comfort working on-site five days per week in downtown Jacksonville.

Comfort working in a large hospital or campus environment with significant walking throughout the day.

Training and Support
BAYADA provides a robust, in-depth training program designed to help you build confidence and succeed in the role. Your onboarding will include approximately 90 days of field-based training and ramp-up, partnership with a Regional Sales Trainer, and ongoing support from Transitional Care Managers and Marketing Managers throughout the local market.
Schedule and Location
This is an on-site position based in the downtown Jacksonville hospital and campus environment, Monday through Friday. The role requires regular movement throughout a large campus, including approximately 2–5 miles of walking per day. Candidates should be comfortable commuting into downtown Jacksonville and working directly within the hospital environment.
