What You’ll Do
As a Behavioral Health RN, you will:
Assess patients’ physical, behavioral, psychosocial, and environmental needs.
Develop, implement, and update individualized plans of care.
Manage an assigned caseload and coordinate services with physicians, caregivers, facilities, and the interdisciplinary care team.
Complete initial evaluations, ongoing assessments, and follow-up visits based on each patient’s needs.
Monitor changes in condition and communicate concerns promptly to the appropriate care team members.
Educate patients, families, and caregivers about behavioral health needs, medications, symptom management, and the plan of care.
Provide clinical guidance and supervision to LPNs and home health aides as appropriate.
Maintain accurate, timely clinical documentation.
Identify and report safety concerns or other issues that may affect patient care.
What You’ll Need
Current, unrestricted Massachusetts RN license
Associate degree in Nursing required; bachelor’s degree preferred
At least one year of nursing experience in a clinical care setting
Behavioral health nursing experience preferred
Home health or hospice experience preferred
Homecare HomeBase experience preferred
Ability and willingness to travel throughout the Worcester/Leicester service area
Dependable vehicle, valid driver’s license, and current auto insurance
Ability to lift, push, pull, support, or transfer up to 50 pounds
