Recovery Coordinator - (Opiates Team)

Location: Forward Leeds, Armley Court, Leeds, LS14 6PA

Working Hours: 37 hours per week

Contract Type: Permanent (Full Time)

Salary: £25,877.80 to £34,240.03 per annum pro rata

Please note: The salary listed reflects the full earning potential for this role. Starting salaries depend on experience and progression within the band.

About the Role

We're looking for a Recovery Coordinator to join our Kirkgate Active Team, supporting adults receiving treatment for opiate use.

This is an opportunity to work with people throughout their recovery journey, building meaningful therapeutic relationships and helping individuals achieve lasting, positive change.

Based at our Armly Court site, you'll become part of a supportive and experienced multidisciplinary team, delivering high-quality treatment and recovery support to people with complex needs. You’ll manage your own caseload while working closely with colleagues and partner agencies to provide coordinated, person-centred care.

What You'll Do

  • Manage a caseload of adults receiving treatment for opiate use, developing, implementing and reviewing personalised recovery plans.
  • Complete triage, screening and comprehensive assessments, tailoring interventions to individual needs and goals.
  • Build trusting, therapeutic relationships that encourage engagement, reduce harm and promote sustained recovery.
  • Work collaboratively with prescribing clinicians and partner agencies to coordinate care and provide integrated support.
  • Help people access healthcare, housing, employment, family support and community resources to strengthen their recovery.
  • Maintain accurate records of recovery plans, safeguarding, risk management and clinical interventions using SystmOne.
  • Contribute to a positive and supportive team while delivering high-quality, person-centred care.

What You'll Bring

We're looking for someone who is compassionate, motivated and passionate about supporting people to achieve meaningful change.

You'll ideally have experience managing a caseload within health, social care or substance use services, alongside knowledge or experience of supporting people affected by alcohol and/or drug use, particularly opiate dependence.

You'll also bring strong communication and relationship-building skills, experience supporting people with complex needs and the ability to work independently while contributing positively to a multidisciplinary team. A relevant Health and Social Care qualification, or equivalent experience, would also be beneficial
